观察复方米非司酮配伍米索前列醇终止早期妊娠的临床疗效

Clinical efficacy of compound mifepristone with misoprostol in the termination of early pregnancy

目的:探究复方米非司酮配伍米索前列醇终止早期妊娠的临床疗效。方法:将2015年8月至2018年1月在昆山市千灯人民医院自愿要求终止妊娠的120例早孕妇女作为研究对象,并根据入院先后实施分组,对照组60例,餐前2h空腹给予复方米非司酮配伍米索前列醇药物流产;观察组60例,清晨空腹给予复方米非司酮配伍米索前列醇药物流产,对两组孕妇用药后孕囊排出时间、出血量、出血时间等临床指标、流产效果以及不良反应发生情况进行综合评价。结果:观察组孕妇用药后孕囊排出时间、流产后24h出血量及出血时间均较对照组短,差异具有统计学意义(P <0. 05);观察组孕妇用药后完全流产率、不全流产率及清宫率分别为86. 7%(52/60)、10. 0%(6/60)、13. 3%(8/60),与对照组呈现差异差异具有统计学意义(P <0. 05);且两组孕妇用药后均伴随轻微腹泻、发热、恶心呕吐等不良反应,未给予特殊处理均自行消失,两组不良反应比较差异无统计学意义(P> 0. 05)。结论:复方米非司酮配伍米索前列醇终止早期妊娠,操作简单,能够促进孕囊排出,减少出血量,安全性高,值得推广应用。

Objective:To investigate the clinical effect of compound mifepristone with misoprostol in the termination of early pregnancy.Methods:120 pregnant volunteers in our hospital for the termination of early pregnancy from January 2015 to January 2017 were selected as the research objects.According to the admission time,they divided into control group and observation group,with 60 cases in each group.Control group was given compound mifepristone with misoprostol for medical abortion(two hours beforo breakfast),while observation group was given compound mifepristone with misoprostol for medical abortion(beforo breakfast).The pregnancy sac discharge time,blood loss,bleeding time,the effect of abortion,adverse reaction and other clinical indexes of two groups were observed.Results:The blood loss and bleeding time of observation group were shorter than those of control group,with statistically significant difference(P<0.05).The complete abortion rate,incomplete abortion rate and curettage rate of observation group after administration was 86.7%(52/60),10.0%(6/60)and 13.3%(8/60)respectively,which were significantly better than those of control group(P<0.05).The mild diarrhea,fever,adverse reactions such as nausea and vomiting,which were not given special treatment,of two groups had no significant difference(P>0.05).Conclusions:Compound mifepristone with misoprostol in the termination of early pregnancy is simple,easy and safe for operation,which can promote the excretion of pregnancy sac and reduce the amount of bleeding,worthy of clinic application.
