慢性肾衰竭男性患者血液透析后性功能障碍的影响因素研究

Influencing factors of sexual dysfunction after hemodialysis in male patients with chronic renalfailure

目的:探讨慢性肾衰竭男性患者血液透析后性功能障碍的影响因素。方法:选择2015年3月至2017年3月湖北文理学院附属襄阳市中心医院收治的慢性肾衰竭的血液透析患者141例为研究对象,根据有无性功能障碍将其分为观察组和对照组。观察组由有性功能障碍102例患者组成,对照组由无性功能障碍的39例患者组成。两组患者均行血液透析治疗;有关调查内容包括年龄、糖尿病史、吸烟史、透析时间、血清白蛋白、血肌酐、尿素氮、体内平滑肌功能异常、血管病变、性激素水平(包括FSH、T、PRL)。单因素和多因素分析慢性肾衰竭男性患者血液透析后性功能障碍独立危险因素。结果:观察组102例患者中,性功能障碍重度39例、中度45例、轻度18例。两组患者年龄、糖尿病史、吸烟史、透析时间及体内平滑肌功能异常比较,其差异均有统计学意义(均P <0. 05);而两组患者白蛋白水平、血肌酐水平、尿素氮水平、FSH水平、T水平及PRL水平比较,差异均无统计学意义(均P> 0. 05);将单因素分析具有统计学意义的纳入多因素Logistic回归分析,年龄、糖尿病史、吸烟史、透析时间和体内平滑肌功能异常为影响慢性肾衰男性患者竭血液透析后性功能障碍的独立危险因素。结论:影响慢性肾衰竭男性患者血液透析后性功能障碍因素中,年龄、糖尿病史、吸烟史、透析时间和体内平滑肌功能异常为其独立危险因素。

Objective:To investigate the influencing factors of sexual dysfunction after hemodialysis in male patients with chronic renal failure.Methods:141 hemodialysis patients with chronic renal failure in Xiangyang Central Hospital Affiliated to Hubei College of Arts and Sciences from March 2015 to March 2017 were selected,including 102 patients with sexual dysfunction(observation group)and 39 patients without sexual dysfunction(control group).All the patients underwent hemodialysis treatment,and their age,history of diabetes,smoking history,duration of dialysis,serum albumin,serum creatinine,urea nitrogen,in vivo smooth muscle dysfunction,vascular disease,sex hormone levels(including FSH,T,PRL)were investigated.Univariate and multivariate analysis of independent risk factors for sexual dysfunction in men with chronic renal failure after hemodialysis was conducted.Results: Among the 102 patients in the observation group,there were 39 cases of severe sexual dysfunction,45 cases of moderate sexual dysfunction and 18 cases of mild sexual dysfunction.Differences in age,history of diabetes,smoking history,duration of dialysis and in vivo smooth muscle dysfunction between the two groups were statistically significant(all P<0.05).There was no significant difference between the two groups in albumin,serum creatinine,urea nitrogen the level of FSH level,T level,and the level of PRL(all P>0.05).According to the multivariate Logistic regression analysis,age,history of diabetes,smoking history,duration of dialysis and in vivo muscle dysfunction were the independent risk factors of sexual dysfunction in hemodialysis patients with chronic renal failure after.Conclusions:Age,diabetes history,smoking history,duration of dialysis and in vivo smooth muscle dysfunction are the independent risk factors of sexual dysfunction after hemodialysis in male patients with chronic renal failure.
