多层螺旋CT增强检查对支气管内膜结核及肺癌病变管壁长度的评价

Evaluation of thel length of wall in endobronchial tuberculosis and lesions of lung cancer by multislice CT enhancement

目的探讨多层螺旋CT(MSCT)增强检查对支气管内膜结核及肺癌病变管壁长度的评价,同时评估CT诊断的要点。方法回顾性选取2014年1月至2018年8月辽宁省铁岭市中心医院收治的支气管内膜结核患者共60例作为研究组,选择同期辽宁省铁岭市中心医院收治的肺癌(中央型)患者共60例作为对照组。对两组患者予以多层螺旋CT增强扫描并给予重建处理,观察病变支气管管壁长度以及CT表现。结果两组患者经CT检查清晰显示共计92段病变支气管。研究组患者支气管管壁平均长度为(31. 66±19. 87) mm,明显长于对照组患者的(16. 27±11. 86)mm,差异具有统计学意义(P <0. 05); CT检查结果显示,与对照组相比,研究组患者的受累支气管部位软组织肿块、纵隔、肺门旁淋巴结肿大不伴钙化以及胸膜增厚表现差异具有统计学意义(P <0. 05)。结论多层螺旋CT增强检查比较支气管内膜结核与肺癌支气管管壁长度,并结合CT表现,有助于提高对以上两种疾病的鉴别诊断,对临床治疗与研究有重要价值。

Objective To explore the evaluation of multi-slice spiral CT(MSCT)enhanced scanning on the length of wall of endobronchial tuberculosis and lung cancer lesions,and to evaluate the main points of CT diagnosis.Methods 60 patients with endobronchial tuberculosis admitted to this hospital during January 2014 to August 2018 were selected as study group.Sixty patients with lung cancer(central type)admitted to this hospital were selected as control group.In both groups,MSCT enhanced scanning and reconstruction were performed in these 2 groups to observe the length of lesions in bronchial wall and CT manifestations.Results Patients of these two groups showed a total of 92 segments of bronchial lesions by CT scanning.The mean length of bronchial wall in study group was(31.66+19.87)mm,it was significantly longer than that of control group(16.27+11.86)mm,with statistically significant difference(P<0.05).The results of CT scanning showed that there were significant differences between the involved bronchial soft tissue masses,mediastinum and pulmonary portal lymph nodes with no calcification and pleural thickening between study group and control group(P<0.05).Conclusion In comparison with the length of bronchial wall between endobronchial tuberculosis and lung cancer by enhanced multi-slice spiral CT scanning is helpful in improving the differential diagnosis of above two kinds of lesions,and it is important for clinical treatment and study.
