脑白质高信号与急性缺血性脑卒中患者功能预后的相关性研究 被引量:5

摘要 目的探究脑白质高信号(WMH)与急性缺血性脑卒中患者预后的相关性。方法回顾性分析2016年6月至2017年10月徐州医科大学附属医院收治的280例急性缺血性脑卒中患者的头颅MRI影像学资料,观察分析不同部位WMH在不同时间点(病后4周、6周、6个月)与急性缺血性脑卒中患者功能预后的相关性。结果脑室旁白质高信号(PVH)与病后4、6周的改良Rankin量表(mRS)评分不相关,与病后6个月的mRS评分相关。深部白质高信号(DWMH)与病后4周、6周及6个月的mRS评分不相关。结论 WMH与急性缺血性脑卒中预后有相关性,PVH与DWMH在预测急性缺血性脑卒中患者的预后中存在差异,病后6个月的PVH与急性缺血性脑卒中患者功能预后有关联。 Objective To explore correlation between white matter high signal( WMH) and prognosis of patients with acute ischemic stroke. Methods Cerebral MRI imaging data of 280 patients with acute ischemic stroke admitted to our hospital from June 2016 to October 2017 were retrospectively collected and analyzed. Correlation of WMH and acute ischemic stroke functional outcome at different sites were observed at different time points( 4 weeks,6 weeks and 6 months after the disease). Results Periventricular hyperintensities( PVH) were associated with mRS scores at 6 months after disease but were not associated at 4 and 6 weeks after disease. Deep white matter hyperintensity( DWMH) was not associated with mRS scores at 4 weeks,6 weeks,and 6 months after illness. Conclusion WMH is associated with prognosis of acute ischemic stroke. PVH and DWMH are different in predicting acute ischemic stroke prognosis. PVH 6 months after surgery is associated with functional prognosis in patients with acute ischemic stroke.
