摘要
目的:探讨动态监测声门与人工气道气囊(artificial airway cuff,AAC)之间腔隙内CO_2体积分数(dynamic monitor of subglottic carbon dioxide levels,DMS-CO_2)技术对监测AAC密闭性的有效性,为AAC压力调控提供指导。方法:以AAC体外实验模型为平台,AAC压力以每次2 cmH_2O(1 cmH_2O=98.06 Pa)的幅度从0 cmH_2O开始逐渐增加至30 cmH_2O。在每个AAC压力水平将美蓝稀释液注入至声门与AAC之间腔隙内,然后监测声门下CO_2体积分数(以CO_2分压表示)、AAC压力和美蓝稀释液渗漏量。结果:在一定范围内声门下CO_2体积分数与AAC压力呈负相关,与AAC渗漏量呈正相关。结论:DMS-CO_2技术可实现AAC密闭性的动态、准确监测,从而验证了DMS-CO_2技术作为AAC密闭性监测手段的可行性和有效性。
Objective To explore the effectiveness of dynamic monitoring of subglottic carbon dioxide levels(DMS-CO2)in the space between glottis and artificial airways cuff(AAC),and to provide guidance for the pressure regulation of AAC.Methods The pressure of AAC in vitro was gradually increased from 0 cmH2O(1 cmH2O=98.06 Pa)to 30 cmH2O with the amplitude of 2 cmH2O.Methylene blue diluent was injected into the space between glottis and AAC at each AAC pressure level,and then the indexes were monitored such as the concentration of subglottic CO2,the pressure of AAC and the leakage of methylene blue diluent.Results In a certain range,the concentration of subglottic CO2 was negatively correlated with the pressure of AAC,and was positively correlated with the amount of leakage.Conclusion DMS-CO2 verifies its feasibility and effectiveness for dynamic and accurate monitoring of AAC sealing.
