脐带延迟结扎对足月新生儿近期及远期健康影响

Effects of Delayed Ligation of Umbilical Cord on Short-term and Long-term Health of Term Neonates

目的:研究脐带延迟结扎对足月新生儿近期以及远期健康情况的影响。方法:纳入温州市中心医院妇产科自2016年6月～2018年1月期间收治的正常足月生产的孕妇600例,随机平均分成3组:A组在足月儿出生时立即断脐;B组在足月儿出生3min后断脐;C组在足月儿出生至脐带搏动消失后断脐。对比3组新生儿的出生后5min Apgar评分、黄疸指数(出生48h后黄疸以及72h后黄疸)以及6月后的血红蛋白以及血细胞比容之间的差异。结果:3组新生儿的Apgar评分以及出生后48h、72h黄疸差异无统计学意义,但出生后1d和6月后B组和C组的血红蛋白水平以及血细胞比容均较A组高,差异有统计学意义,而B组和C组之间差异无统计学意义。结论:脐带延迟结扎能提升足月新生儿的近期和远期健康能起到提高新生儿血红蛋白水平以及血细胞比容,有效预防新生儿贫血。

Objective:To study the effect of delayed ligation of umbilical cord on short-term and long-term health of term neonates.Methods:600 cases of normal full-term pregnant women treated in a hospital from June 2016 to January 2018 were selected and randomly,equally divided into three groups:Group A received umbilical cord ligation immediately when the term infants were born;Group B received umbilical cord ligation in 3 minutes and Group C received umbilical cord ligation when the pulse of umbilical disappeared.The difference of 5-minute infant Apgar score,icteric index(48h postnatal icteric and 72h postnatal icteric)and the level of hemoglobin and hematocrit value after 6 months among three groups were compared.Results:There was no significant difference in Apgar score and jaundice at postnatal 48h and 72h among the three groups.The hemoglobin level and hematocrit in group B and group C were significantly higher than those in group A at 1 day after birth and at 6 months after birth,but there was no significant difference between group B and group C.Conclusion:The delayed ligation of umbilical cord can improve the short-term and long-term health of full-term newborns,which can improve the level of hemoglobin and hematocrit of newborns and prevent neonatal anemia effectively.
