局部亚低温联合改良去大骨瓣减压术对重度颅脑损伤患者术后颅内压及并发症发生率的影响 被引量:7

摘要 目的:探讨改良去大骨瓣减压术联合局部亚低温治疗重度颅脑损伤对患者术后颅内压及并发症发生率的影响。方法:选取2015年3月~2017年3月某院84例重度颅脑损伤患者,按照随机数字表法分组,各42例。对照组给予改良去大骨瓣减压术治疗,观察组采用局部亚低温联合改良去大骨瓣减压术治疗,统计对比两组治疗前后颅内压变化,对比两组并发症发生率。结果:治疗后观察组颅内压较对照组低,差异有统计学意义(P<0.05);观察组并发症发生率7.14%(3/42)低于对照组28.57%(12/42),差异有统计学意义(P<0.05)。结论:局部亚低温联合改良去大骨瓣减压术,可降低重度颅脑损伤患者术后颅内压,并发症少,值得推广。 Objective:To investigate the effect of local mild hypothermia combined with modified decompressive craniectomy on postoperative intracranial pressure and complication rate in patients with severe traumatic brain injury.Methods:84 patients with severe craniocerebral injury in a hospital from March 2015 to March 2017 were randomly divided into the control group and the observation group,42 cases in each group.The control group was treated with modified decompressive craniectomy,while the observation group was treated with local mild hypothermia combined with modified decompressive craniectomy.The changes of intracranial pressure before and after treatment were compared between the two groups,and the incidence of complications was compared between the two groups.Results:After treatment,the intracranial pressure of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05);The complication rate of the observation group was 7.14%(3/42),which was lower than that of the control group 28.57%(12/42),the difference was statistically significant(P<0.05).Conclusion:Local mild hypothermia combined with modified decompressive craniectomy can reduce postoperative intracranial pressure in patients with severe traumatic brain injury,with fewer complications,which is worthy of promotion.
