早发型重度子痫前期合并胎儿生长受限期待治疗的妊娠结局

Pregnancy outcome of expectant treatment for pregnant women with early onset severe preeclampsia complicated with fetal growth restriction

目的探讨早发型重度子痫前期合并胎儿生长受限期待治疗的妊娠结局。方法将早发型重度子痫前期孕产妇98例作为研究资料,其中合并胎儿生长受限42例,作为观察组,未合并胎儿生长受限56例,作为对照组,均行常规期待治疗和对症治疗,比较两组临床资料及妊娠结局。结果两组孕产妇子痫、胎盘早剥、心衰、HELIP综合征、血小板减少、产后出血等并发症比较,P>0.05,无明显差异;两组孕周延长时间比较,P>0.05,无明显差异;观察组终止妊娠时间及新生儿出生体重均显著低于对照组(P<0.05);两组早产、剖宫产等分娩结局比较,P>0.05,无明显差异;观察组新生儿窒息、肺透明膜病变、心脏发育不全等新生儿并发症显著低于对照组(P<0.05);两组新生儿黄疸、新生儿贫血、新生儿死亡等发生情况比较,P>0.05,无明显差异。结论早发型重度子痫前期合并胎儿生长受限虽然给新生儿带来诸多不良影响,但通过期待治疗可保证孕产妇安全,获得良好的妊娠结局。

Objective To explore the pregnancy outcome of expectant treatment for pregnant women with early onset severe preeclampsia combined with fetal growth restriction. Methods 98 pregnant women with early onset severe preeclampsia who were treated in our hospital from June 2015 to June 2017 were selected. The observation group included 42 cases combined with fetal growth restriction. The control group included 56 cases without fetal growth restriction. All subjects were treated with routine expectant treatment. The clinical data and pregnancy outcome were compared between the two groups. Results There were no significant difference in eclampsia, placental abruption, heart failure, HELIP syndrome, thrombocytopenia, and postpartum hemorrhage between the two groups, P>0.05; the two groups had no significant difference in the duration of pregnancy, P>0.05; the pregnancy termination time and the birth weight in the observation group were significantly lower than those in the control group, P<0.05; neonatal asphyxia, pulmonary hyaline membrane disease and cardiac dysplasia in the observation group were significantly lower than those in the control group, P<0.05; there was no significant difference in the incidence of neonatal jaundice, neonatal anemia and neonatal death between the two groups, P>0.05. Conclusion early onset severe preeclampsia combined with fetal growth restriction has many adverse effects on newborns, but expectant treatment can ensure the safety of pregnant women and get a good pregnancy outcome.
