加速康复外科在老年股骨颈骨折全髋关节置换术后的应用价值

Clinical value of fast track surgery in elderly patients with femoral neck fracture after total hip arthroplasty

目的讨论加速康复外科(ERAS)在老年股骨颈骨折全髋关节置换术(THA)后的临床应用价值。方法将在本院行全髋关节置换术的老年股骨颈骨折患者101例,随机分为实验组和对照组。对照组50例采用常规临床干预措施,实验组51例增加ERAS训练。对比两组术后2周、5周视觉模拟评分(VAS)、髋关节Harris评分,并比较两组并发症发生率及锻炼依从性。结果实验组术后2周、5周VAS评分、Harris评分与对照组对比,P<0.05。干预后实验组依从率显著高于对照组,P<0.05。实验组并发症发生率低于对照组,P<0.05。结论 ERAS理念应用于THA术后患者,可有效降低患者术后疼痛度及并发症,加速髋关节功能康复,提高锻炼依从性。

Objective To explore the clinical value of fast track surgery (FTS) in elderly patients with femoral neck fracture after total hip arthroplasty (THA). Methods 101 elderly patients with femoral neck fracture who did total hip arthroplasty in our hospital were randomly divided into the experimental group and the control group. 50 patients in the control group were treated with clinical routine intervention and 51 patients in the experimental group were given FTS training on the basis of the control group. The visual analogue scale (VAS) and hip Harris scores were compared between the two groups 2 and 5 weeks after surgery. The complication rate and exercise compliance were also compared between the two groups. Results The VAS score and Harris score of the experimental group were compared with those in the control group 2 weeks and 5 weeks after operation with P<0.05. After the intervention, the total compliance rate of the experimental group was significantly higher than that of the control group, P<0.05. The total complication incidence in the experimental group was lower than that in the control group, P<0.05. Conclusion The FTS applied to patients with THA can effectively reduce postoperative pain and complications, accelerate hip function and improve exercise compliance.
