宁夏银北地区慢性HBV感染者病毒基因型的分布与临床的关系

Distribution of viral genotypes and clinical relationshipin patients with chronic HBV infection in Yinbei area of Ningxia

目的:研究慢性HBV感染者病毒基因型在宁夏银北地区的分布情况以及与临床的关系。方法:收集慢性HBV感染者乙肝病毒DNA阳性患者81例,包括慢性乙型肝炎、乙肝后肝硬化、原发性肝癌血液标本,检测乙肝病毒血清学标志物、乙肝病毒脱氧核糖核酸、肝功能、Fibroscan无创性超声诊断仪检测肝脏纤维化程度、荧光PCR法测定乙肝病毒基因型。结果:本研究纳入的研究对象感染乙肝病毒基因型为B、C、D、C/D重组型4种,以C型(79%)为主。检测出的4种基因型年龄组分布差异有统计学意义(P<0.05)。C型在肝硬化组、肝癌组的比例明显高于其他基因型,不同基因型间HBV DNA、ALT、AST、肝脏纤维化程度差异无统计学意义(P>0.05),乙肝病毒e抗原阳性分布差异有统计学意义(P<0.05)。结论:慢性HBV感染者在宁夏银北地区病毒基因型分布为B、C、D、C/D重组型4种,以C型为主;C型在肝硬化组、肝癌组的比例明显高于其他基因型;C型、C/D重组型中乙肝病毒e抗原阳性率明显高于其他基因型。

Objective:To study the distribution of viral genotypes and its relationship with clinicalin patients with chronic HBV infection in Yinbei area of Ningxia.Methods: 81 cases of hepatitis B virus DNA positive patients with chronic HBV infection were enrolled,including chronic hepatitis B,post-hepatitis B cirrhosis and primary liver cancer blood specimens;detection of hepatitis B virus serological markers,hepatitis B virus DNA,liver function,Fibroscan non-invasive ultrasound diagnostic instrument for detecting liver fibrosis,fluorescent PCR method for determination of hepatitis B virus genotype.Result:The subjects included in this study were infected with hepatitis B virus genotypes as B,C,D,and C/D recombinants,C type (79%).The differences in the distribution of the four genotypes detected in the age group were statistically significant (P<0.05).The proportion of type C in liver cirrhosis group and liver cancer group was significantly higher than other genotypes.There was no significant difference in HBV DNA,ALT,AST and liver fibrosis between different genotypes (P>0.05),the positive distribution of hepatitis B virus e antigen was statistically significant (P<0.05).Conclusion:The distribution of viral genotypes in patients with chronic HBV infection in Yinbei area of Ningxia is B,C,D,C/D recombinant, typeC is the most;The proportion of type C in liver cirrhosis group and liver cancer group was significantly higher than other genotypes;The positive rate of hepatitis B virus e antigen in type C and C/D recombinants was significantly higher than other genotypes.
