hook-wire联合弹簧圈技术在肺孤立性结节术前CT定位的应用价值 被引量:1

摘要 目的:探讨CT引导下hook-wire联合弹簧圈术前定位在孤立性肺小结节(SPN)患者行楔形胸腔镜切除术中的应用价值。方法:27例(31个)体检发现SPN患者,平均大小约0.9±0.12cm。随机分为A、B两组,A组16例(19枚)SPN患者肺小结节行hook-wire联合弹簧圈定位,B组为对照组,11(13枚)例SPN患者肺小结节行单纯hook-wire定位,回顾性分析两组操作成功率、操作时间、位移脱钩率,统计学行分组卡方检验比较两组逐项指标之间的差异。结果:两组技术操作成功率均为100%。操作时间A组13.2±2.4分钟,B组14.5±1.8分钟,两组比较2=1.32,P=4.28,结果无统计学差异。并发症比较分别为:A组气胸15.78%(3/19)、B组15.38%(2/13),两组比较?2=0.47,P=5.42;出血15.38%(3/19)、23.07%(3/13);两组比较2=3.21,P=1.37,结果无统计学差异。A组无位移脱钩现象,B组有2例发生位移及脱钩,转开胸手术,脱钩位移率为15.38%,两组比较2=22.47,P=0.001,结果具有统计学差异。结论:SPN胸腔镜术前肺小结节CT引导穿刺下hook-wire联合弹簧圈定位率高,有效降低定位失败风险,并发症少,操作相对简单,值得推广。
