超前镇痛用于无痛人工流产术后疼痛管理的临床效果观察

Observation of the effect of preemptive analgesia on pain management of patients after painless induced abortion

目的:评价氟比洛芬酯超前镇痛在无痛人工流产术后疼痛管理的效果。方法:选择2017年8月在本院计划生育门诊行无痛人工流产术的患者共计219例作为研究对象,两组手术开始前10min均开放静脉输注乳酸钠林格氏液,超前镇痛组给予氟比洛芬酯100mg,两组均给予丙泊酚与瑞芬太尼复合麻醉,对者的术后疼痛视觉模拟评分(VAS),初步评价超前镇痛在无痛人工流产术后疼痛管理的临床效果。结果:实验组手术时间为3.8±1.1min,对照组手术时间为4.5±2.0min,差异存在统计学意义(P<0.05);实验组术后10min、20min、30min、24hVAS评分分别为:0.0(0.0,1.0)分、0.0(0.0,1.0)分、0.0(0.0,0.0)分、0.0(0.0,0.0)分。对照组术后10min、20min、30min、24hVAS评分分别为:1.0(0.0,2.0)分、1.0(0.0,2.0)分、0.0(0.0,1.0)分、1.0(0.0,2.0)分,实验组各个时间段内VAS评分均低于对照组,差异有统计学意义(P<0.05)。结论:超前镇痛能有效减轻无痛人工流产术后疼痛,是一种安全的、有效的镇痛模式值得推广。

Objective:To evaluate the effect of preemptive analgesia with flurbiprofen axetil on pain management of patients after painless induced abortion.Methods:219 patients who experienced painless induced abortion in Beijing obstetrics and gynecology hospital in August 2017 were selected as the subjects of this study.The patients in preemptive analgesia group and in control group were all given combined anesthesia with propofol and remifentanil for surgical abortion.Before abortion,sodium lactate Green’s solution were infused intravenously 10 minutes before operation in both groups,and 100 mg flurbiprofen ester was added in the preemptive analgesia group.The visual analogue scale(VAS)of postoperative pain of patients was compared between the two groups,and the clinical effect of preemptive analgesia on pain management of patients after painless induced abortion was preliminarily evaluated.Results:The operation time of patients in preemptive analgesia group was 3.8±1.1 min,while that of patients in control group was 4.5±2.0 min,which had statistically significant different(P<0.05).The VAS scores of patients in preemptive analgesia group at 10 min,20 min,30 min and 24 hafter operation were 0.0(0.0,1.0),0.0(0.0,1.0),0.0(0.0,0.0),0.0(0.0,0.0),respectively,and the VAS scores of patients in control group at 10 min,20 min,30 min and 24 hafter operation were1.0(0.0,2.0),1.0(0.0,2.0),0.0(0.0,1.0),1.0(0.0,2.0),respectively.The VAS score of patients in preemptive analgesia group at different time was significant lower than that of patients in control group(P<0.05).Conclusion:Preemptive pain can effectively reduce pain of patients after painless induced abortion,which is a safe and effective mode of analgesia for clinical popularization and application.
