沙丁胺醇联合布地奈德在支气管哮喘急性发作期患者中的治疗效果 被引量:3

摘要 目的观察沙丁胺醇联合布地奈德治疗支气管哮喘急性发作期的临床效果。方法选择2016年1月—2017年12月期间该院收治的支气管哮喘急性发作期患者60例,采用随机数字表法分为对照组、研究组。对照组在止咳平喘、祛痰及抗感染等常规治疗基础上加用沙丁胺醇,研究组在对照组基础上联合布地奈德雾化吸入治疗,观察两组治疗效果、肺功能指标[前后用力肺活量(FVC)、第1用力呼吸容积(FEV1)、峰值呼气流速(PEF)]、肿瘤坏死因子-α(TNF-α)及血清C反应蛋白(CRP)水及不良反应发生情况。结果研究组治疗总有效率96.67%,高于对照组的73.33%,差异有统计学意义(χ~2=4.706,P<0.05);两组治疗后FVC、FEV1、PEF等肺功能指标均较本组治疗前改善,且研究组改善情况优于对照组,差异有统计学意义(t=4.272、4.412、4.376,P<0.05);研究组治疗后TNF-α及CRP明显低于对照组,差异有统计学意义(P<0.05);对照组治疗期间不良反应总发生4例(13.33%),研究组不良反应总发生2例(6.67%);两组不良反应发生率对比,差异无统计学意义(χ~2=0.185,P>0.05)。结论沙丁胺醇联合布地奈德雾化吸入治疗支气管哮喘急性发作期效果显著,利于改善患者肺功能及血清TNF-α及CRP水平,安全性较高,值得推广应用。 Objective To observe the clinical effect of salbutamol combined with budesonide in the treatment of patients with bronchial asthma at acute stage. Methods 60 cases of patients with bronchial asthma at acute stage admitted and treated in our hospital from January 2016 to December 2017 were selected and randomly divided into two groups, the control group used the salbutamol on the basis of the routine therapy, while the research group used the atomization inhalation of salbutamol combined with budesonide on the basis of the control group, and the treatment effect, lung function indexes such as FVC, FEV1, PEF, TNF-α, CRP levels and occurrence of adverse reactions were observed. Results The total effective rate in the research group was higher than that in the control group(96.67% vs 73.33%), and the difference was statistically significant(χ^2=4.706, P<0.05); and the FVC)FEV1)PEF of the two groups were improved compared with those in the two groups, and the improvement in the research group was better than that in the control group, and the difference was statistically significant(t=4.272,4.412,4.376, P<0.05); and the TNF-α, CRP in the research group after treatment were obviously lower than those in the control group, and the differences were statistically significant(P<0.05), and there were 4 cases with adverse reactions during the treatment in the control group(13.33%), and there were 2 cases with adverse reactions(6.67%), and the difference in the incidence rate of adverse reactions between the two groups was not statistically significant (χ^2=0.185, P>0.05). Conclusion The effect of salbutamol combined with budesonide in the treatment of patients with bronchial asthma at acute stage is obvious, which is conductive to improving the lung function and serum TNF-α及CRP levels of patients, with higher safety, and it is worth promotion and application.
