摘要
目的研究分析俯卧位呼吸机通气在重症肺炎患儿抢救中的应用效果。方法选取2016年5月—2018年5月在该院就诊的重症肺炎患儿120例,采用随机数表法,将其随机均分为观察组(60例)和对照组(60例)。对照组患儿采用常规仰卧位呼吸机通气,观察组采用俯卧位呼吸机通气,两组患儿均采用保护性通气模式。治疗两周后,对比分析两组患儿不同时段的血气分析,以及两组患儿治疗有效率。结果观察组在6、12、24 h的PaO_2变化分别为(89.9±2.1)、(91.2±7.2)、(92.1±8.2)mmHg,与对照组的(88.7±9.1)、(88.1±7.7)、(90.2±8.0)mmHg,比较差异有统计学意义(P<0.05);观察组在6、12、24 h的PaO2/FiO2变化分别为(145.4±5.2)、(151.9±6.5)、(183.4±8.6),与对照组的(121.1±5.5)、(127.4±7.3)、(151.5±7.8),比较差异有统计学意义(P<0.05);观察组患儿治疗有效率(91.67%),明显高于对照组的有效率(40.00%)。结论俯卧位呼吸机通气可明显提高通气效果,有利于气道分泌物引流,且治疗效果较好,值得推广应用。
Objective To investigate the application effect of prone position respirator ventilation in the rescue of severe patients with pneumonia.Methods 120 cases of children with severe pneumonia diagnosed in our hospital from May 2016 to May 2018 were selected and randomly divided into two groups with 60 cases in each,the control group and the observation group respectively used the routine prone position respirator ventilation and supine position respirator ventilation,and all children used the protective ventilation model,after two weeks,the blood gas analysis of children in the two groups at different time periods was compared,and the treatment effective rate of the two groups was analyzed.Results The PaO2 changes at 6,12,24 h in the observation group and in the control group were respectively(89.9±2.1),(91.2±7.2),(92.1±8.2)mmHg and(88.7±9.1),(88.1±7.7),(90.2±8.0)mmHg,and the differences were statistically significant(P<0.05),the PaO2/FiO2 changes at 6h,12h,24h in the observation group and in the control group were respectively(145.4±5.2),(151.9±6.5),(183.4±8.6)and(121.1±5.5),(127.4±7.3),(151.5±7.8),and the differences were statistically significant(P<0.05),and the treatment effective rate in the observation group was obviously higher than that in the control group(91.67%vs 40.00%).Conclusion The prone position respirator ventilation can obviously improve the ventilation effect,which is conductive to the airway secretion drainage,and the treatment effect is better,and it is worth promotion and application.
