用动脉血替代静脉血做全血细胞分析的可行性分析

目的探讨用动脉血代替静脉血做全血细胞分析的可行性。方法选取2018年4—5月在该院住院的患者30例为研究对象,对每个患者在同一时间分别抽取静脉血(EDTA-K3抗凝)及动脉血(0.2 mL的6 250 U/mL肝素钠)。使用日本SYSMEX公司的XS-800i全血细胞分析仪对静脉血(EDTA-K3抗凝)及动脉血(0.2 mL的6 250 U/mL肝素钠)做全血细胞分析检测,并进行统计学分析,比较用动脉血(0.2 mL的6 250 U/mL肝素钠)和静脉血(EDTA-K3抗凝)的白细胞计数(WBC)、红细胞计数(RBC)、血红蛋白(HGB)、血小板计数(PLT)、红细胞压积(HCT)、白细胞分类等项目的相关性。结果使用动脉血(0.2 mL的6 250 U/mL肝素钠)做全血细胞分析检测与静脉血(EDTA-K3抗凝)主要项目结果的相关系数:白细胞计数(WBC)0.982、红细胞计数(RBC)0.985、血红蛋白(HGB) 0.983、血小板计数(PLT) 0.975、红细胞压积(HCT) 0.965,除此之外,白细胞分类及红细胞相关项目指标均高度相关,血小板压积(PCT)0.814,相关性较低。结论使用动脉血(0.2 mL的6 250 U/mL肝素钠)可以代替静脉血(EDTA-K3抗凝)做全血细胞分析检测,但血小板相关指标仅供临床参考。

Objective To study the feasibility of arterial blood replacing venous blood for whole blood cell test.Methods 30 cases of inpatients in our hospital from April 2018 to May 2018 were selected as the research objects and the venous blood(EDTA-K3 anticoagulant)and arterial blood(0.2 mL 6 250 U/mL heparin sodium)of each patients at the same time were studied,and the venous blood(EDTA-K3 anticoagulant)and arterial blood(0.2 mL 6 250 U/mL heparin sodium)were for whole blood cell analysis and test by the XS-800i whole blood cell analyzer from Japan SYSMEX Company,and the patients were for statistical analysis,and the correlation between the arterial blood(0.2 mL 6 250 U/mL heparin sodium)and venous blood(EDTA-K3 anticoagulant),WBC,RBC,HGB,PLT,HCT and leukocyte differential count was compared.Results The related coefficients of major projects results of arterial blood(0.2 mL 6 250 U/mL heparin sodium)and venous blood(EDTA-K3 anticoagulant):WBC 0.982,RBC 0.985,HGB 0.983,PLT 0.975,HCT 0.965,besides,the leukocyte differential count was highly related to red blood cell related items,and the PCT was 0.814,and the correlation was low.Conclusion The arterial blood(0.2 mL 6 250 U/mL heparin sodium)can replace the venous blood(EDTA-K3 anticoagulant)for whole blood cell test and analysis,but the platelet related indicators can only used for clinical reference.
