血清超敏C反应蛋白、癌胚抗原、糖类抗原724和糖类抗原199检测对胃癌及其相关病变的早期诊断价值 被引量:16

摘要 目的探讨血清超敏C反应蛋白(hs-CRP)、癌胚抗原(CEA)、糖类抗原724(CA724)、糖类抗原199(CA199)检测对胃癌及其相关病变的早期诊断价值。方法采用免疫比浊法和电化学发光法对就诊于新疆自治区人民医院胃癌病人86例、中重度异型增生的病人34例、萎缩性胃炎的病人63例、健康人(对照组) 70例血清hs-CRR、CEA、CA724和CA199水平进行检测。结果胃癌组血清hs-CRP、CEA、CA724和CA199水平及阳性检出率均高于萎缩性胃炎组及对照组(P <0. 01)。中重度异型增生组病人血清hs-CRP和CA724较胃癌组低(P <0. 01),血清hs-CRP、CEA和CA199较萎缩性胃炎组及对照组高(P <0. 05)。胃癌病人hs-CRP、CEA、CA724和CA199与肿瘤的发生部位无关(P> 0. 05)。胃癌病人低分化组血清hs-CRP、CA724和CA199水平高于高分化组及中分化组(P <0. 05);中分化组血清hs-CRP水平高于高分化组(P <0. 01)。4种指标联合检测的敏感性和准确性均高于单项检测(P <0. 05)。结论联合检测血清hs-CRP、CEA、CA724和CA199对胃癌的早期诊断、治疗及预后评估有重要价值。 Objective To investigate the correlation between high sensitive C reaction protein(hs-CRP),carcinoembryonicantigen(CEA),carbohydrateantigen 724(CA724),carbohydrateantigen 199(CA199)and gastric cancer and its associated lesions and to evaluate the clinical significance of early diagnosis of the proteins.Methods A total of 86 cases of gastric cancer,86 cases of moderate-severe hyperplasia,63 cases of atrophic gastritis and 70 cases of healthy individuals were enrolled in this study.Immunoturbidimetry was used to detect hs-CRP.Electrochemiluminescence was used to detect CEA,CA199 and CA724.Results Serum hs-CRP,CEA,CA724,CA199 and its positive rate of GC patients were statistically higher than those in atrophic gastritis and healthy individuals(P<0.01).Serum hs-CRP,CA724 of moderate-severe hyperplasia were statistically lower than those in GC patients(P<0.01),which higher than those in atrophic gastritis and healthy individuals(P<0.05).For GC patients,serum hs-CRP,CEA,CA724,CA199 were correlated with tumor stages(P<0.05),however,had no relationship with the location of tumors(P>0.05).The diagnostic sensitivity and specificity of combination detection were higher than single detection.Conclusion combination detection of serum hs-CRP,CEA,CA724 and CA199 might had great values in early diagnosis,treatment and prognosis of gastric cancer.
