乳晕去表皮真皮脂肪瓣充填加荷包缝合矫正乳头内陷10例

Treatment of ten cases of inverted nipples by areola of epidermis and dermis fat filling and purse-string suture

目的探讨乳晕去表皮真皮脂肪瓣充填加荷包缝合矫正乳头内陷的临床应用方法和效果。方法 2014 2017年滁州市第一人民医院治疗不同原因导致的乳头内陷16例。观察组10例彻底松解乳头下引起乳头内陷的纤维束和条索带,松解乳头基底部,设计位于乳晕内外侧以乳头根部为蒂的顶点不超过乳晕边缘两菱形真皮脂肪瓣,上提乳头,将该瓣下拉填充于乳头基底部,双重荷包缝合固定。对照组6例只采用乳晕去表皮真皮脂肪瓣充填矫正乳头内陷。结果乳晕去表皮真皮脂肪瓣充填加荷包缝合矫正术10例乳头内陷病人随访半年,乳头在乳晕平面以上≥10 mm。对照组6例病人乳头在乳晕平面以上5～10 mm,其中有2例病人双侧乳头部分回缩。结论乳晕去表皮真皮脂肪瓣充填加荷包缝合术式矫正乳头内陷具有操作简单、效果确切、不易复发和疤痕小等优点,是临床上较为实用的一种方法。

Objective To investigate the clinical application and effect of areola dermal fat flaps filled with purse suture for correction of nipple inversion.Method During the period of 2014 2017,16 patients with inverted nipples caused by different causes were treated in the First People s Hospital.Ten patients in the observation group were thoroughly relieved of the fibers bundles and bands that caused inverted nipples under the nipples and the basal part of the nipples.Two rhombic dermal fat flaps with the apex of the nipple root pedicled on the inside and outside of the nipple areola were designed to lift the nipples.The flap was pulled down and filled in the base of the nipple.The double purse was sutured and fixed.Six cases in the control group were treated with areole dermal fat filling to correct inverted nipples.Result The 10 patients with inverted nipples who undergone areola dermal fat flaps filled with purse suture were followed up for half a year after operation.The height of nipples was increased by at least 10 mm.In the control group,the height of nipples increased by 5~10 mm.Conclusion Areola dermal fat flaps filled with purse suture for correction of nipple inversion is simple,effective,less recurrence and less scar.It is a more practical method in clinic.
