小剂量轻比重腰麻-硬膜外联合麻醉对老年下肢骨折手术患者的作用

Effect of Low-dose Light-weight Spinal Anesthesia Combined with Epidural Anesthesia on Elderly Patients with Lower Extremity Fracture

目的对小剂量轻比重腰麻-硬膜外联合麻醉方法在老年下肢骨折中的临床效果进行探究。方法以该院骨科于2015年7月—2016年7月之间收治的92例下肢骨折的老年患者为研究对象,并将其按麻醉方法的不同分成对照组46例和观察组46例。其中对照组患者使用连续硬膜外麻醉的方式进行治疗,观察组则运用小剂量轻比重腰麻-硬膜外联合麻醉方式进行治疗,对两组患者的麻醉阻滞生效间隔、麻醉阻滞完善时长以及麻醉药物注射剂量等进行观察记录并做对比。结果通过对于观察组的麻醉药的用量、起效的时间长短以及治疗的完善时间等进行统计,发现观察组的麻醉剂的用量为(5.2±0.6)mg,起效时间在(4.7±2.0)s,完善的时间在(6.3±3.5)s,都明显的优于对照组,数据结果比较差异有统计学意义(t=3.28、3.59、3.73 P<0.05)。结论于老年患者的下肢骨折手术中采用小剂量轻比重腰麻-硬膜外联合麻醉的方式不仅能降低麻醉药物注射剂量还能缩减药物起效时长和神经阻滞恢复时间,具有良好的作用,能够作为一种安全性更高效果更好的麻醉方法。

Objective To investigate the clinical effect of low-dose light-weight spinal anesthesia combined with epidural anesthesia in elderly lower extremity fractures. Methods A total of 92 elderly patients with lower extremity fractures who were admitted to the department from July 2015 to July 2016 were enrolled in the study. They were divided into the control group(46 cases) and the observation group(46 cases) according to the anesthesia method. The control group were treated with continuous epidural anesthesia. The observation group was treated with low-dose light-weight spinal anesthesia combined with epidural anesthesia. The anesthesia block was effective and the anesthesia block was perfected. The duration and the dose of anesthetic injection were observed and compared. Results Through the statistics of the amount of anesthetic used in the observation group, the length of onset of action, and the time of treatment, etc., it was found that the anesthetic dosage of the observation group was(5.2±0.6)mg, and the onset time was(4.7±2.0)s. The perfect time was(6.3±3.5)s, which was significantly better than that of the control group. The data were statistically significant(t=3.28,3.59,3.73,P<0.05). Conclusion Low-dose light-weight spinal anesthesia com bined with epidural anesthesia in elderly patients with lower extremity fractures can not only reduce the dose of anesthetic drugs, but also reduce the duration of drug onset and recovery time of nerve block, which has a good effect and can be used as a safer and better anesthesia method.
