某院门诊西药房退药原因分析及应对措施探讨

Analysis of the Reasons for Drug Withdrawal in Western Pharmacy of a Hospital and Its Countermeasures

目的探讨某院门诊西药房退药原因及应对措施。方法选取该院门诊西药房2017年1—12月176例退药处方为研究对象,采用回顾性分析总结分析其退药原因,并且提出相应的对策。结果 176例退药处方退药原因依次为:药品因素86例(48.86%,86/176)(主要为不良反应、近效期药等);医生因素54例(30.68%,54/176)(处方出错、不合理处方、重复开药、疗程过长、处方错误等);患者因素36例(20.45%,36/176)(家中已有所需药品;对药物不认可;患儿不配合使给药困难;不具备储存条件;费用过高;病情变化病情好转或恶化)。结论门诊药房退药原因具有多样性,退药行为难以完全杜绝,通过建立严格的退药制度,规范相关程序,采用有针对性的措施降低药房的退药率。

Objective To explore the reasons for the withdrawal of western pharmacy in a hospital outpatient clinic and the countermeasures. Methods 176 cases of drug withdrawal prescriptions from January to December 2017 were selected as the research subjects. The retrospective analysis was used to analyze the reasons for drug withdrawal, and corresponding countermeasures were proposed. Results The causes of withdrawal of 176 cases of drug withdrawal were as follows: 86 cases of drug factors (48.86%, 86/176)(mainly adverse reactions, near-term drugs, etc.); 54 cases of doctors (30.68%, 54/176) prescription error, unreasonable prescription, repeated prescribing, long course of treatment, prescription error, etc.; 36 patients (20.45%, 36/176) of patients (there is a drug in the family; the drug is not recognized; the child does not cooperate in administration; no storage conditions; high cost; condition changes or worsens). Conclusion There are many reasons for drug withdrawal in outpatient pharmacies, and it is difficult to completely eliminate drug withdrawal behavior. By establishing a strict drug withdrawal system, standardizing relevant procedures, and using targeted measures to reduce the drug withdrawal rate.
