银杏达莫联合小牛血清去蛋白提取物治疗后循环缺血性眩晕临床评价

Clinical Evaluation on Ginkgo Leaf Extract and Dipyridamole Injection Combined with Deproteinized Extract of Calf Blood in the Treatment of Posterior Circulation Ischemic Vertigo

目的探讨银杏达莫联合小牛血清去蛋白提取物治疗后循环缺血性眩晕(PCIV)的疗效及对内皮功能和血流动力学的影响。方法选择医院2014年4月至2018年4月收治的PCIV患者81例,随机分为对照组(40例)和研究组(41例)。两组患者均行常规治疗,并给予小牛血清去蛋白提取物,研究组患者加用银杏达莫,均治疗10 d。结果研究组总有效率为95.12%,显著高于对照组的77.50%(P <0.05);治疗后,两组患者内皮素-1 (ET-1)、血管性血友病因子(vWF)及血栓调节蛋白(TM)均明显降低,且研究组明显低于对照组(P <0.05);降钙素基因相关肽(CGRP)水平及两侧椎动脉和椎-基底动脉血流速度均升高,且研究组明显高于对照组(P <0.05);两组不良反应发生率相当(4.88%比2.50%,P> 0.05)。结论银杏达莫联合小牛血清去蛋白提取物治疗PCIV,疗效确切,对患者的内皮功能、血流动力学均有明显改善作用,且不增加不良反应,具有一定的临床应用价值。

Objective To investigate the clinical effect of Ginkgo Leaf Extract and Dipyridamole Injection combined with deproteinized extract of calf blood(DECB) in the treatment of posterior circulation ischemic vertigo(PCIV) and its effect on endothelial function and hemodynamics.Methods Totally 81 patients with PCIV admitted to our hospital from April 2014 to April 2018 were selected and randomly divided into the control group(40 cases) and the study group(41 cases).The patients in the two groups received routine treatment and DECB,on this basis,the patients in the study group were treated with Ginkgo Leaf Extract and Dipyridamole Injection.All patients were treated for 10 d.Results The total effective rate of the study group was 95.12%,which was significantly higher than77.50% of the control group(P < 0.05).After treatment,the endothelin-1(ET-1),von Willebrand factor(vWF) and thrombomodulin(TM) in the two groups were significantly decreased,and those in the study group were significantly lower than those in the control group(P < 0.05).The level of calcitonin gene-related peptide(CGRP) and the blood flow velocity of both sides vertebral and vertebrobasilar arteries were increased,and those in the study group were significantly higher than those in the control group(P < 0.05).The incidence rate of adverse reactions in the two groups was similar(4.88% vs.2.50%,P > 0.05).Conclusion Ginkgo Leaf Extract and Dipyridamole Injection combined with DECB in the treatment of PCIV has a definite curative effect.It can significantly improve the endothelial function and hemodynamics of patients without increasing adverse reactions,which has certain clinical application value.
