醋酸亮丙瑞林预处理用于腹腔镜下子宫肌瘤剔除术临床评价

Clinical Evaluation on Pretreatment with Leuprorelin Acetate for Laparoscopic Myomectomy

目的探讨醋酸亮丙瑞林术前预处理用于腹腔镜下子宫肌瘤剔除术的临床疗效及对局部微循环的影响。方法选取医院2016年8月至2017年12月收治的子宫肌瘤患者120例,随机分为研究组与对照组,各60例。两组患者均给予腹腔镜下子宫肌瘤剔除术治疗,研究组患者手术前给予醋酸亮丙瑞林预处理。结果研究组患者用药后肌瘤和子宫体积均显著减小(P <0.05);研究组患者术中出血量较对照组显著减少,盆腔引流时间、下床活动时间、住院时间较对照组显著缩短(P <0.05);两组患者手术时间比较无显著差异(P> 0.05);研究组患者手术后微循环指标(除毛细血管管径外)较手术前变化程度不显著(P <0.05),但与对照组同时间点比较均有显著差异(P <0.05)。结论腹腔镜下子宫肌瘤剔除术前应用醋酸亮丙瑞林预处理效果较好,可有效改善围术期指标,且对局部微循环影响较小。

Objective To investigate the clinical effect of pretreatment with leproproline acetate before laparoscopic myomectomy(LM)and its effect on local microcirculation.Methods Totally 120 patients with uterine fibroids admitted to our hospital from August 2016 to December 2017 were selected and randomly divided into the study group and the control group,60 cases in each group.The patients in the two groups were treated with LM,the patients in the study group were pretreated with leuprorelin acetate before the operation.Results In the study group,the volume of uterine fibroids and the volume of uterus decreased after the treatment(P < 0.05).During the operation,the amount of intraoperative bleeding in the study group was significantly lower than that in the control group,the time of pelvic drainage,ambulation and hospitalization was significantly shorter than that of the control group(P < 0.05),but there was no significant difference in operation time between the two groups(P > 0.05).After the operation,the changes of postoperative microcirculation indexes(except capillary diameter) in the study group were not significant compared with those before operation(P > 0.05),but there were significant differences between the two groups at the same time points(P < 0.05).Conclusion The pretreatment with leproproline acetate before LM can effectively improve the perioperative indexes and has little effect on local microcirculation.
