结直肠腺瘤切除术后复发情况及相关影响因素分析

Analysis of postoperative recurrence and related influencing factors of colorectal adenoma resection

目的研究结直肠腺瘤切除术后复发情况及其影响因素。方法选取2011-04至2013-04就诊于解放军总医院第三医学中心的120例结直肠腺瘤患者,行腺瘤切除术后进行随访。对复发患者的腺瘤复发数量、大小、位置、病理分型及复发时间进行记录,同时记录其年龄、性别、相关病史,分析与复发相关的危险因素。结果共112例患者完成随访,其中复发患者69例,复发率为61.6%。经多因素Logistic回归分析显示,吸烟、饮酒、年龄≥60岁、糖尿病史、高代谢综合征、腺瘤数量≥3个为结直肠腺瘤复发的危险因素(P<0.05),病理分型为管状腺瘤、药物史与患者息肉复发呈负相关(P<0.05)。结论结直肠腺瘤切除术后复发率较高,影响因素较多,应给予更多指导与监控。

Objective The objective of this study was to explore the postoperative recurrence of colorectal adenoma and its influencing factors. Methods 120 patients with colorectal adenoma treated with adenoma resection in the Third Medical Center of Chinese PLA General Hospital from April 2011 to April 2013 were followed up. The number, size, location, pathological classification and recurrence time of adenomas in relapsed patients were recorded. Age, sex and related medical history were recorded, and the influencing factors of recurrence were analyzed. Results A total of 112 patients completed the follow-up review, of whom 69 relapsed, with a recurrence rate of 61.6%. Multivariate Logistic regression analysis showed that smoking, drinking, age( ≥ 60 years), diabetes history, hypermetabolic syndrome, and adenoma number(≥3) were risk factors for recurrence of colon adenoma(P<0.05); pathological classification of tubular adenoma and drug history were negatively correlated with polyp recurrence(P<0.05). Conclusions The recurrence rate of colorectal adenoma after resection is high, and it has many influencing factors. Thus more guidance and monitoring should be given to it.
