益气养血化痰通络方对一氧化碳中毒后迟发性脑病血清免疫相关因子及大脑核磁共振成像的影响

Effects of Yiqi Yangxue Huatan Tongluo recipe on serum immune factors and brain magnetic resonance imaging in patients with delayed encephalopathy after carbon monoxide poisoning

目的观察益气养血化痰通络方对一氧化碳中毒后迟发性脑病(DEACMP)患者血清免疫相关因子和大脑核磁共振成像的影响。方法将100例DEACMP患者按照随机数字表法分为2组,对照组50例予西医常规治疗,治疗组50例在对照组治疗基础上加益气养血化痰通络方治疗。比较2组治疗前后血清白细胞介素4(IL-4)、IL-10、干扰素γ(IFN-γ)及转化生长因子β_1(TGF-β_1)水平及侧脑室周围白质、半卵圆中心和苍白球区平均扩散系数(ADC)值,脑白质病变分级情况,统计临床疗效。结果治疗组总有效率96. 00%,对照组总有效率74. 00%,治疗组高于对照组(P <0. 05)。治疗后,2组IL-4、IL-10、TGF-β_1水平均较本组治疗前升高(P <0. 05),IFN-γ水平降低(P <0. 05),且2组组间比较差异均有统计学意义(P <0. 05)。治疗后,2组侧脑室周围白质、半卵圆中心区及苍白球ADC值均较本组治疗前提高(P <0. 05),且治疗组高于对照组(P <0. 05)。2组治疗后脑白质病变分级均较本组治疗前改善(P <0. 05),且治疗组改善优于对照组(P <0. 05)。结论益气养血化痰通络方治疗DEACMP能够提高临床疗效,改善机体神经免疫炎症反应,减轻脑组织损伤。

Objective To observe the clinical effects of Yiqi Yangxue Huatan Tongluo recipe on serum interleukin-4(IL-4),interleukin-10(IL-10),interferon-γ(IFN-γ)and transforming growth factor-β1(TGF-β1)levels and brain magnetic resonance imaging in patients with delayed encephalopathy after carbon monoxide poisoning(DEACMP).Methods 100 patients with DEACMP were divided into two groups according to the random number table method.50 cases in control group were treated by routine western medicine.50 cases in treatment group were treated by Yiqi Yangxue Huatan Tongluo recipe on the basis of control group.The serum levels of IL-4,IL-10,IFN-γ,TGF-β1,the average diffusion coefficient(ADC)values of periventricular white matter,semioval center and globus pallidus,the grading of cerebral white matter lesions before and after treatment were compared between the two groups.Results The total effective rate was 96.00%in treatment group and 74.00%in control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the levels of IL-4,IL-10 and TGF-β1 in the two groups were higher than those in the group before treatment(P<0.05),and the levels of IFN-γwere decreased(P<0.05).There was significant difference between the two groups(P<0.05).After treatment,the ADC values of periventricular white matter,globus pallidus and semioval center of the two groups were increased compared with the group before treatment(P<0.05),and the treatment group was higher than the control group(P<0.05).After treatment,the grading of white matter lesions in the two groups was better than that of the group before treatment(P<0.05),and the improvement in the treatment group was better than that in the control group(P<0.05).Conclusion The treatment of DEACMP with Yiqi Yangxue Huatan Tongluo recipe can improve the clinical efficacy,improve the body's neuroimmune inflammatory response and alleviate brain tissue damage.
