摘要
目的了解老年股骨颈骨折患者行全髋关节置换术(THA)术后体重的变化。方法回顾性分析2013年1月至2016年6月在聊城市第二人民医院关节外科接受单侧初次THA的股骨颈骨折患者。以BMI为分组依据,18. 5 kg/m2≤BMI <24. 0 kg/m2为体重正常组,24. 0 kg/m2≤BMI <28. 0 kg/m2为超重组,BMI≥28. 0 kg/m2为肥胖组。采用Kaplan-Meier法分析比较THA后人体质量指数(BMI)减少超过5%的时间间隔。结果共有78例患者纳入本研究,体重正常组28例患者、超重组46例患者、肥胖组4例患者,3组患者之间的年龄、性别构成差异无统计学意义,P> 0. 05。3组患者在THA术后人体质量指数减少超过5%的中位时间分别为52个月(43,69)个月、48个月(40,67)个月、47个月(41,60)个月,行log-rank检验,差异没有统计学意义(X^2=4. 72,P> 0. 05)。结论体重正常组、超重组、肥胖组患者在THA术后BMI减少超过5%的中位时间基本一致。
Objective To investigate the changes of body weight after total hip arthroplasty(THA)in elderly patients with femoral neck fracture.Methods The patients received a unilateral primary THA with femoral neck fracture from January 2013 to June 2016 in the department of orthopaedics,the Second People’s Hospital of Liaocheng,were analyzed.Patients were divided into 3 groups according to BMI,18.5 kg/m^2≤BMI<24.0 kg/m^2 for normal weight group,24.0 kg/m^2≤BMI<28.0 kg/m^2 for overweight group,and BMI≥28.0 kg/m^2 for obesity group.The Kaplan-Meier method was used to analyze the time interval in which the body mass index(BMI)decreased by more than 5%after THA.Results A total of 78 patients were enrolled in the study.There were 28 patients in the normal weight group,46 patients in the overweight group,and 4 patients in the obese group.There was no significant difference in age and gender between the three groups(P>0.05).The median survival time to decrease the body mass index by more than 5%after THA in the three groups was 52 months(43,69)months,48 months(40,67)months,47 months(41,60)months respectively.A log-rank test was performed(χ^2=4.72,P>0.05),indicating that the difference was not statistically significant.Conclusion In the normal weight group,the overweight group and the obese group,the median survival time of BMI reduction of more than 5%after THA was basically the same.
