踝关节后外侧“烟斗样”切口的临床应用

Clinical application of the "posterolateral" incision of the ankle joint

目的观察踝关节后外侧"烟斗样"切口同时处理后踝、外踝、下胫腓前下韧带的损伤的临床疗效。方法选取上海市第一人民医院2015年1月至2016年6月收治的107例累及后踝的旋后外旋3°和旋前外旋4°踝关节骨折住院手术患者。通过踝关节后外侧"烟斗样"切口同时处理外踝、后踝骨折及下胫腓前下韧带损伤。结果所有患者手术切口愈合良好,下胫腓前韧带、外踝、后踝均得到满意的处理。结论踝关节后外侧"烟斗样"切口通过一个切口、两个间隙同时处理下胫腓前韧带、外踝及后踝,视野清晰,操作简便,适合处理外旋型踝关节骨折伴有后外侧后踝骨折患者。

Objective To explore effect of post-lateral "pipe-like" incision in treating injuries of the posterior ankle, lateral ankle and the anterior inferior ligaments at the same time. Methods A total of 107 cases of ankle fracture involving posterior malleolus treated in our hospital from January 2015 to June 2016 were selected. A"pipe-like" incision at the posterolateral part of the ankle was applied to treat fracture of lateral ankle, posterior ankle and injury of anterior inferior tibiofibular ligament. Results All the patients healed well, and the anterior tibiofibular ligament, lateral malleolus and posterior malleolus were all satisfactorily managed. Conclusion The "pipe-like" incision at the posterolateral part of the ankle can treat the anterior tibiofibular ligament, lateral malleolus and posterior malleolus at the same time through one incision and two spaces. It has clear vision and simple operation. It is suitable for the treatment of patients with lateral malleolus fracture accompanied by posterolateral malleolus fracture.
