双手托下颌法对无痛胃镜检查的影响

The effect of mandibular lift on painless gastroscopy

目的:探讨双手托下颌法对无痛胃镜检查的影响。方法:选取接受无痛胃镜的受检者100例,采用随机数字法分为对照组和观察组,每组50例。对照组受检者麻醉后行胃镜检查,观察组麻醉后行双手托下颌,然后行胃镜检查。记录并比较两组受检者各时间点的生命体征和出现呛咳、体动反应、呼吸抑制、咽喉不适、追加丙泊酚的情况以及检查者和受检者的满意度。结果:两组受检者在T2和T3的MAP、HR、Sp O2和RR均有不同程度的下降(P <0. 05或P <0. 01),观察组受检者在T3时的MAP、HR、Sp O2和RR低于对照组,差异有统计学意义(P <0. 05);观察组受检者呛咳、体动反应、呼吸抑制的发生率低于对照组,差异有统计学意义(P <0. 05),检查者对观察组的满意度高于对照组,差异有统计学意义(P <0. 05)。结论:双手托下颌法在无痛胃镜检查中可以减轻受检者呛咳反应、体动反应和呼吸抑制。

Objective To observe the effect of mandibular lift on painless gastroscopy.Method 100 cases of painless gastroscopy were selected,and they were randomly divided into control group and observation group,50 cases in cach group.patients in control group were received gastroscopy after anesthesia,patients in observation group were mandibular lift after anaesthesia,and then received gastroscopy.The vital signs at each time point and choking cough,body reaction,respiratory depression,throat discomfort,addition of propofol,patients'and doctors'satisfaction in two groups were recorded and compared.Results The MAP,HR,SpO2 and RR were decreased at T2 and T3 than T1 in two groups(P<0.05 or P<0.01),the MAP,HR,SpO2 and RR in observation group were lower at T3 than that in control group(P<0.05).The incidence of cough,body movement and respiratory depression in observation group was lower than that in control group(P<0.05).The satisfaction of doctor in observation group was higher than that in control group(P<0.05).Conclusion Mandibular lift can relieve the patient's cough,body movement and respiratory depression in the painless gastroscopy.
