剪切波弹性成像联合超声造影在BI-RADS Ⅲ～Ⅳ级乳腺肿块中的应用价值

Application of Shear Elastography Combined with Contrast-enhanced Ultrasound in Breast Masses of BI-RADS Grade Ⅲ～Ⅳ

目的:探讨剪切波弹性成像(SWE)联合超声造影(CEUS)在BI-RADSⅢ～Ⅳ级乳腺肿块的应用价值。方法:选取超声诊断患有乳腺肿块,并接受超声定位下细针穿刺活检术或手术治疗的女性患者。利用随机数表法分成两组。对照组50例,采用常规超声检查;试验组50例,采用SWE联合CEUS检查。计算各组的敏感度、特异度、准确率、阳性预测值与阴性预测值,比较两组的准确率、敏感度及特异度。结果:试验组敏感度、特异度、准确率(96. 67%、80. 00%、90. 00%)明显优于对照组(88. 89%、56. 52%、74. 00%),差异有统计学意义(P <0. 05)。结论:切波弹性成像联合超声造影对BI-RADSⅢ～Ⅳ级乳腺肿块诊断效能高,可以更准确评估肿物的良恶性,相对早期准确定性,值得临床推广。

Objective To investigate the value of shear wave elastography(SWE)combined with contrast-enhanced ultrasound(CEUS)in the diagnosis of BI-RADSⅢ~Ⅳbreast masses.Method The patients were selected in our hospital who were diagnosed breast tumors,and patients were admitted to ultrasound-targeted fine-needle aspiration biopsy or surgery.They were divided into two groups using the random number table method.50 patients were in the control group,using routine ultrasonography.50 patients were in the test group,using SWE combined CEUS.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of each group were calculated,and the accuracy,sensitivity and specificity of the two groups were compared.Results The sensitivity,specificity and accuracy of the test group(96.67%,80.00%,90.00%)were significantly better than those of the control group(88.89%,56.52%,74.00%)(P<0.05).Conclusion The combination of shear wave elastography and contrast-enhanced ultrasonography has high diagnostic efficacy for BI-RADSⅢ~Ⅳbreast masses.It can more accurately evaluate benign and malignant tumors,Accurately determine the nature early relatively.It is worthy of clinical promotion.
