大肠癌临床辨证面诊信息特征研究

Study of Face Information Features about Differentiation of Clinical Syndrome in Colorectal Cancer Patients

目的:分析大肠癌患者及不同证候的面诊信息的变化特点。方法:运用上海中医药大学自行研制的DKF-Ⅱ型中医面诊数字化检测仪检测493例大肠癌患者的面诊客观参数;统计方法运用t检验、方差分析。结果:(1)大肠癌组的红色指数、青色指数、黑色指数比正常人高(P <0. 05),白色指数和黄色指数比正常人低(P <0. 05);面部有光泽指数比正常人低(P <0. 05);唇色S比正常组高(P <0. 05),明度I比正常组低(P <0. 05)。(2)大肠癌各证型面信息参数之间存在差异:气血两虚组的红色指数比其他组低,与脾虚气滞组有显著差异(P <0. 05);湿热蕴结组的白色指数比其他组低,与脾肾阳虚组有显著差异(P <0. 05)。五个证型在面部光泽指数和唇色指数间无显著差异。结论:面诊客观参数可以作为大肠癌辨证的辅助参考指标,进而为临床医生提供规范化的辅助诊疗手段。

Objective: To analyze the changing characteristics of the face information of patients with colorectal cancer with different syndromes. Methods: The clinical face objective parameters of 493 patients with colorectal cancer were examined by DF-Ⅱ TCM Digital Diagnostic Instrument developed by Shanghai University of Traditional Chinese Medicine,and statistical methods using t test,variance analysis. Results:( 1) In colorectal cancer group,the index of red,blue and black was higher than that of normal( P < 0. 05),and the index of white and yellow was lower than that of normal( P < 0. 05); facial luster index was lower than that of normal( P< 0. 05); lip gloss index S was higher than that of the normal group( P < 0. 05),lightness I was lower than that of normal( P < 0. 05).( 2) There were differences among the various types of colorectal cancer in the face information parameters,red index in qi and blood deficiency group was lower than that of other groups,and there was a significant difference compared to spleen deficiency and qi stagnation group( P < 0. 05); white index in accumulated damp-heat knot was lower than that of other groups,and there was a significant difference compared to spleen and kidney yang deficiency group( P < 0. 05). There was no significant difference among the 5 syndromes in the face and lip gloss index. Conclusion: The objective face parameters of diagnosis can be used as a secondary reference index of colorectal cancer syndrome differentiation,and then to provide clinicians with standardized methods of diagnosis and treatment.
