经腹超声联合经会阴超声对前置胎盘的诊断价值

Diagnostic value of transabdominal ultrasound combined with perineal ultrasound in placenta previa

目的探讨经腹超声联合经会阴超声对前置胎盘诊断的临床应用价值。方法 129例疑似前置胎盘孕妇先后进行经腹超声和经会阴超声检查,以产后观察情况作为确诊标准,比较单一检查和联合检查的临床诊断价值。结果联合检查对前置胎盘的检出率(60例,96.77%)以及疑似前置胎盘诊断准确率(125例,96.90%)均明显高于经腹超声和经会阴超声检查,差异有统计学意义(P<0.017);联合检查对前置胎盘诊断的灵敏度(96.77%)、特异度(97.01%)、约登指数(0.938)、阳性预测值(96.77%)和阴性预测值(97.01%)均较高,其中联合检查的灵敏度高于经腹超声和经会阴超声检查,阳性预测值高于经腹超声检查,差异均有统计学意义(P<0.017)。结论经腹超声联合经会阴超声可提高前置胎盘临床诊断及分型的准确率,应用价值较高。

Objective To explore the clinical application value of transabdominal ultrasound combined with transperineal ultrasound in the diagnosis of placenta previa.Methods 129 pregnant women with suspected placenta previa were examined by transabdominal ultrasound and transperineal ultrasound successively,and postpartum observation was used as diagnostic criteria.And then,the clinical diagnostic value of single examination and combined examination were compared.Results The detection rate of placenta previa(60 cases,96.77%)and the diagnostic accuracy of suspected placenta previa(125 cases,96.90%)by combined examination were significantly higher than those by transabdominal ultrasound examination and transperineal ultrasound examination,difference was statistically significant(P<0.017).The sensitivity(96.77%),specificity(97.01%),Yorden index(0.938),positive predictive value(96.77%)and negative predictive value(97.01%)of combined examination for placenta previa were higher.The sensitivity of combined examination was significantly higher than transabdominal ultrasound examination and transperineal ultrasound examination,and the positive predictive value was significantly higher than that of transabdominal ultrasound examination,difference was statistically significant(P<0.017).Conclusion The examination of transabdominal ultrasound combined with transperineal ultrasound can improve the accuracy of clinical diagnosis and classification of placenta previa,which has high application value.
