小肠间质瘤44例临床治疗及预后分析

Clinical therapy and prognosis analysis of 44 cases of small intestinal stromal tumors

目的:探讨小肠间质瘤临床治疗方法及影响预后的相关危险因素。方法:回顾性分析2006年1月—2010年10月南京市第一医院手术证实的小肠间质瘤44例,对所有病例进行随访,将影响肿瘤复发及预后的可能相关因素,进行分级变量单因素和连续变量多因素分析,筛出影响小肠间质瘤预后危险因素。结果:患者临床复发与年龄、间质瘤危险度和肝转移显著相关(P <0.001),而临床表现、CD117、CD34和靶向治疗与肿瘤复发无显著相关(P>0.05),不同临床表现组间累积生存率无显著差异。对上述临床因素多因素分析发现,肿瘤复发与年龄、靶向治疗、肝转移、危险度因素显著相关(P <0.05),而与临床表现、CD117、CD34无显著相关(P>0.05)。结论:手术仍是小肠间质瘤治疗的首选。

Objective:To Investigate the therapeutic method and related risk factor affecting on prognosis of the small intestinal stromal tumors.Method:44 patients with stromal tumors of small intestine confirmed surgically between January 2006 and October 2010 were followed up.Classified and continuous variable such as age,different manifestation,neoplasma risk,expressions of CD117 and CD34,liver metastasis,targeted therapy were analyzed to identify the factors affecting the relapse and prognosis.Results:Clinical relapse is significant correlation to the age,risk factor and liver metastasis(P<0.001)instead of clinical features,expression of CD117 and CD34,and targeted therapy(P>0.05)by Single Factor Analysis.Accumulative survival rate hasn’t significant deviation between different clinical feature(P>0.05).It is found that tumor relapse is significant correlation to the age,targeted therapy,liver metastasis,risk factors(P<0.05)instead of clinical feature,expression of CD117 and CD34(P>0.05).Conclusion:Surgical procedure is of choice for intestinal stromal tumor.Imatinib has a good effect in reducing the postoperative recurrence rate in patients with advanced small intestinal stromal tumors.
