自拟益肾健脾活血方联合西药治疗原发性骨质疏松症疗效及对BALP、TRACP-5b水平的影响

Effect of self-developed Yishen Jianpi Huoxue recipe combined with western medicine in the treatment of primary osteoporosis and its influence on BALP and TRACP-5b levels

目的探讨自拟益肾健脾活血方联合西药治疗原发性骨质疏松症疗效及对骨碱性磷酸酶(bone alkaline phosphatase,BALP)、抗酒石酸酸性磷酸酶5b(tartaric acid acid phosphatase 5b,TRACP-5b)水平的影响。方法研究对象选取我院2015年2月～2017年6月收治的原发性骨质疏松症患者共112例,以随机数字表法分为对照组(n=56)和观察组(n=56),分别采用唑来膦酸+碳酸钙D3和在此基础上辅以自拟益肾健脾活血方治疗;比较两组患者治疗前后视觉模拟评分(visual analogue scale,VAS)、Oswestry功能障碍指数(Oswestry disability index,ODI评分)、骨密度值、血清Ca、P、BALP、TRACP-5b水平及不良反应发生率。结果观察组患者治疗后的VAS评分和ODI评分均显著低于对照组和治疗前(P<0.05);观察组患者治疗后骨密度值显著高于对照组和治疗前(P<0.05);两组患者治疗前后血清Ca和P水平比较差异无显著性(P>0.05);观察组患者治疗后血清BALP和TRACP-5b水平均显著低于对照组和治疗前(P<0.05);同时两组患者不良反应发生率比较差异无显著性(P<0.05)。结论自拟益肾健脾活血方联合西药治疗原发性骨质疏松症能够显著缓解疼痛症状,改善肢体活动功能,提高骨密度,调节血清BALP和TRACP-5b水平,且安全性值得认可。

Objective To explore the efficacy of self-developed Yishen Jianpi Huoxue Recipe combined with Western medicine in the treatment of primary osteoporosis and its effect on bone alkaline phosphatase(BALP) and tartaric acid phosphatase 5b(TRACP-5)levels.Methods A total of 112 patients with primary osteoporosis who were admitted in our hospital from February 2015 to June 2017 were selected as research subjects.They were divided into control group(n=56) and observation group(n=56) by random number table method.Zoledronic acid+calcium carbonate D3 was used respectively,and on the basis of this,self-made Yishen Jianpi Huoxue Recipe was given;visual analogue scale(VAS),Oswestry disability index(ODI score),bone mineral density,serum Ca,P,BALP,TRACP-5b levels and incidence rate of adverse reactions were compared between the two groups.Results The VAS score and ODI score in the observation group were significantly lower than those in the control group and before treatment(P<0.05);the bone mineral density in the observation group after treatment was significantly higher than that in the control group and before treatment(P<0.05);there was no significant difference in serum Ca and P levels between the two groups before and after treatment(P>0.05);the serum BALP and TRACP-5b levels in the observation group after treatment were significantly lower than those in the control group and before treatment(P<0.05);at the same time,there was no significant difference in the incidence rate of adverse reactions between the two groups(P<0.05).Conclusion Self-developed Yishen Jianpi Huoxue Recipe combined with western medicine in the treatment of primary osteoporosis can significantly relieve pain symptoms,improve limb function,increase bone density,regulate serum BALP and TRACP-5b levels,and the safety is worthy of recognition.
