急诊外科以护士为主体的疼痛管理模式应用研究

Application of nurse-led pain management program in the department of emergency surgery

目的探讨建立一种有效合理的急诊外科疼痛管理模式,以提高疼痛管理质量,防止患者因疼痛造成的生理、心理影响。方法将200名我院急诊外科住院患者按照随机数字表法分为对照组和观察组各100例,对照组采用急诊外科常规护理方法,患者在有疼痛主诉时护士根据医嘱进行处理。观察组开展以护士为主体的疼痛管理模式,成立疼痛管理小组,对护士进行系统化的疼痛管理培训,护士对患者适时镇痛、制定个体化的疼痛管理措施,并依照疼痛评估标准记录疼痛程度。结果观察组术后12、24,48,72 h疼痛评分低于对照组,夜间睡眠质量良好率及镇痛满意度明显高于对照组,两组比较差异有统计学意义(P <0. 05)。结论以护士为主体的疼痛管理模式适用于急诊外科患者,充分发挥了多元化镇痛的优势,有效地减轻了患者的疼痛,改善夜间睡眠,提高患者的满意度。

Objective To set-up an effective and reasonable management program for pain management in the department of emergency surgery so as to improve the quality of pain management in order to prevent the physiological and psychological effects caused by pain.Methods Two hundred inpatients of emergency surgery in our hospital were randomly divided into control or observation group,100 in each group.The control group was given routine nursing in emergency surgery,and the patients were treated according to the doctor's advice when there were complaints of pain.The observation group carried out a nurse-led pain management program.Here,a pain management team was set up to train nurses systematically on pain management.In the program,nurses gave timely analgesia and individualized pain management measures,and recorded the pain levels according to pain assessment criteria.Results The good quality rate of nocturnal sleep and the satisfaction degree of analgesia in the observation group were significantly higher than that in the control group and the dosage of opioid drugs was reduced(P<0.05).Conclusions The nurse-led pain management program is suitable for the patients in the emergency surgery.It exerts the advantage of multiple analgesias,effectively alleviates the pain of the patients,and improves the night sleep and the patient's satisfaction.
