十一味益肾降糖片治疗早期糖尿病肾病的临床疗效观察

Clinical therapeutic effects of shiyiwei yishen jiangtang tablets on early diabetic nephropathy

目的观察十一味益肾降糖片治疗早期糖尿病肾病的临床疗效。方法将气阴两虚兼瘀型早期糖尿病肾病患者60例,随机分为对照组和治疗组。对照组30例,在常规治疗上选用厄贝沙坦片口服。治疗组30例,在对照组治疗的基础上加用十一味益肾降糖片口服。连续治疗12周,比较两组临床疗效,观察治疗前后中医证候积分、空腹血糖(FBG)、糖化血红蛋白(Hb A1C)、尿微量白蛋白(MALB)、尿白蛋白排泄率(UAER)、血清胱抑素C(Cys-C)、血清转化生长因子β1(TGF-β1)的变化。结果总有效率治疗组为83. 3%,对照组为56. 7%,治疗组疗效优于对照组(P <0. 05)。治疗组在降低中医证候积分方面明显优于对照组(P <0. 05)。两组患者FBG、Hb A1c水平较治疗前无明显下降(P> 0. 05)。治疗组患者MBAL、UAER、Cys-C、TGF-β1水平的下降幅度优于对照组(P <0. 05)。结论十一味益肾降糖片治疗DKD安全有效,可以显著降低尿蛋白,保护肾功能,减轻临床症状,改善生活质量及延缓肾衰进程,值得进一步推广。

Objective To observe the clinical therapeutic effects of shiyiwei yishen jiangtang tablets on early diabetic nephropathy(DN). Methods A total of 60 patients of early DN differentiated as qi and yin deficiency and blood stagnation were randomized into a control group and a treatment group. In the control group,30 cases were treated with the routine medication with the oral administration of irbesartan tablets. In the treatment group,on the base of the treatment as the control group,30 cases were treated with shiyiwei yishen jiangtang tablets. The medication was given consecutively for12 weeks. The clinical effects were compared and TCM symptom score,FBG,Hb A1 C,MALB,UAER,Cys-C and TGF-β1 were observed before and after treatment in the two groups. Results The total effective rate was 83. 3% in the treatment group and was 56. 7% in the control group. The therapeutic effect in the treatment group was better than the control group(P < 0. 05). Regarding the reducing TCM symptom scores,the result in the treatment group was obviously better than the control group(P < 0. 05). The levels of FBG and Hb A1 C after treatment were not reduced obviously as compared with the levels before treatment in the two groups(P > 0. 05). The reducing amplitude of MALB,UAER,Cys-C and TGF-β1in the treatment group was better than the control group(P > 0. 05). Conclusion Shiyiwei yishen jiangtang tablets are safe and effective in the treatment of DKD. The drug significantly reduces urine protein,protects kidney function,alleviates the clinical symptoms,improves the quality of life and delays the progress of renal failure. Hence,this drug deserves to be promoted.
