头穴丛刺与语言康复训练联合奥瑞姆自理模式治疗脑卒中运动性失语的临床疗效分析

Clinical therapeutic effects on apoplectic aphemia treated with the cluster needling of scalp acupuncture,speech rehabilitation and Orem self-care mode

目的观察头穴刺激法与语言训练联合奥瑞姆自由模式在运动性失语患者中的临床疗效评价。方法收集符合临床研究标准的运动性失语患者106例,随机分为对照组50例与治疗组56例。对照组给予头穴丛刺激+语言训练+神经科常用药物治疗,治疗组在对照组的基础上给予奥瑞姆自理模式。两组均连续治疗3个月。治疗后评价两组患者的语言能力情况(采用北京医院汉语语言检查法、失语商指数、功能性语言沟通能力检查法、失语症严重程度分级标准)。结果治疗后治疗组总有效率高于对照组(P <0. 05),治疗前两组在语言功能评分、失语商指数、功能性语言沟通能力评分、失语严重程度分级上比较两组差异均无统计学意义(P> 0. 05)。治疗后治疗组在语言功能评分、失语商指数、功能性语言沟通能力评分、失语严重程度分级上均优于对照组(P <0. 05)。结论在头穴丛刺与语言康复训练的基础上给予奥瑞姆自理模式是一种很好的治疗运动后失语的临床治疗方案。

Objective To evaluate the clinical therapeutic effects on aphemia treated with scalp acupuncture,speech training and Orem self-care mode. Methods A total of 106 cases of aphemia eligible to clinical criteria were randomized into a control group(50 cases) and a treatment group(56 cases). In the control group,the cluster needling stimulation of scalp acupuncture,speech training and common medicines of neurology department were applied. In the treatment group,on the base of the treatment as the control group,Orem self-care mode was supplemented. The treatment was given consecutively for 3 months in the two groups. After treatment,the speech ability was evaluated with the Chinese language test of Beijing Hospital,aphasia quotient index,functional communication ability test and the grading criteria of aphasia severity. Results After treatment,the total effective rate in the treatment group was higher than the control group(P <0. 05). Before treatment,there were no differences in the linguistic function score,aphasia quotient index,functional communication score and aphasia severity grade between the two groups(P > 0. 05). After treatment,the linguistic function score,aphasia quotient index,functional communication score and aphasia severity grade in the treatment group were better than the control group(P < 0. 05). Conclusion On the base of the cluster needling of scalp acupuncture and speech training,the intervention of Orem self-care mode is the satisfactory therapeutic regimen for aphemia.
