加速康复外科理念在腹腔镜结直肠手术患者围手术期护理中的应用

Application of enhanced recovery after surgery concept in perioperative nursing of patients undergoing laparoscopic colorectal surgery

目的分析加速康复外科(ERAS)理念在腹腔镜结直肠手术患者围手术期护理中的应用效果。方法选取2017年8月—12月在广东省第二人民医院进行结直肠手术患者26例,设为传统组,围手术期予以常规护理;选取2018年1月—5月进行结直肠手术患者22例,设为ERAS组,围手术期采用ERAS理念进行护理。观察2组手术相关指标、术后管道留置情况,以及术后并发症发生情况。结果 ERAS组术后首次排气时间、排便时间、下床时间、住院总天数、进水时间及进食时间均优于传统组,且术后留置胃管、尿管、引流管的天数明显短于传统组,差异有统计学意义(P <0.05)。2组术后并发症发生率比较差异无统计学意义(P> 0.05)。结论对结直肠手术患者围手术期应用ERAS理念可加速术后康复,缩短住院时间,且不增加术后并发症的发生。

Objective To explore the application effect of enhanced recovery after surgery(ERAS) concept in the perioperative nursing of patients undergoing laparoscopic colorectal surgery.Methods Twenty-six patients undergoing laparoscopic colorectal surgery from August,2017 to December,2017(application of routine nursing in the perioperative period) were collected as traditional group; 22 patients undergoing laparoscopic colorectal surgery from January,2018 to May,2018(application of ERAS concept nursing in the perioperative period) were collected as ERAS group.The surgery-related indexes,postoperative tube indw elling and incidence of complications were observed between two groups.Results When compared with traditional group,the first exhaust time,defecation time,time to get out of bed,total hospital stays,time to drink water and take food as well as postoperative indwelling days of gastric tube,urine tube and drainage tube were all improved in ERAS group,and the significant differences were shown(P < 0.05).There was no statistical significance between two groups regarding the incidence of postoperative complications(P > 0.05).Conclusion Application of ERAS concept in the perioperative nursing of patients undergoing laparoscopic colorectal surgery can shorten hospital stays and accelerate the postoperative rehabilitation,without the increase of postoperative complications.
