上肢运动预防PICC置管患者静脉血栓的Meta分析 被引量:6

A meta-analysis of preventive effect of upper limb movement on the phlebothrombosis in the patients undergoing peripherally inserted central catheter

摘要 目的评价上肢运动对经外周置入中心静脉导管(PICC)置管患者静脉血栓的预防效果。方法计算机检索中国生物医学文献数据库(CBM)、中国知网全文数据库(CNKI)、万方数据库、维普数据库、Pub Med数据库、CINAHL数据库、Cochrane图书馆和JBI图书馆,搜集符合纳入及排除标准的随机对照试验,对文献质量进行评价后,采用RevMan 5.3软件分析数据。结果共纳入6篇符合标准的文献,Meta分析结果显示:上肢运动能有效降低PICC置管患者静脉血栓的发生率[OR=0.18,95%CI(0.09,0.37),P <0.00001]; PICC置管患者进行上肢运动能有效改善腋静脉平均血流速度[WMD=1.56,95%CI(0.72,2.39),P=0.0003]。结论上肢运动可有效降低PICC置管患者静脉血栓的发生率,改善腋静脉平均血流速度。 Objective To evaluate the preventive effect of upper limb movement on the phlebothrombosis in the patients undergoing peripherally inserted central catheter(PICC).Methods Various Chinese and English databases were retrieved by the computer,and the randomized controlled trials meeting the inclusion and exclusion criteria were collected.The data were analyzed through RevMan 5.3 software after evaluation on the quality of articles.Results Totally 6 articles were included.Meta-analysis results revealed that upper limb movement could effectively decrease the incidence of phlebothrombosis in the patients undergoing PICC [OR = 0.18,95% CI(0.09,0.37),P < 0.00001].The results of 4 studies displayed that upper limb movement could effectively improve the mean flow velocity of axillary vein in the patients undergoing PICC[WMD = 1.56,95% CI(0.72,2.39),P = 0.0003].Conclusion Upper limb movement can effectively decrease the incidence of phlebothrombosis and improve the mean flow velocity of axillary vein in the patients undergoing PICC.
