中心静脉导管在肾脏及肾周微创引流中的应用

The evaluation of the application of central venous catheter in kidney and perirenal minimally invasive drainage

目的:探讨中心静脉导管在肾脏及肾周微创引流中的应用价值。方法:收集我院近3年来因各种原因引起的肾积水53例和肾脓肿32例需行肾造瘘术引流的患者,随机分成中心静脉导管治疗组(简称导管组:肾积水28例;肾积脓15)和传统肾造瘘治疗组(简称传统组:肾积水25例;肾积脓17例)进行对比研究,同时回顾分析应用中心静脉导管引流的其他少数病例(肾周积脓、多囊肾出血后合并感染、肾周积血吸收不良、肾结核性脓肿)的治疗效果。结果:分别对比分析肾积水,肾脓肿两组手术时间,术中出血,术后疼痛时间,结果显示导管组与传统组对比均有统计学意义(P﹤0.05);其他少数病例应用中心静脉导管引流也取得了令人满意的治疗效果。结论:中心静脉导管因操作简单,创伤小,费用低,临床依从性高,治疗效果满意等优点值得在泌尿外科诊疗过程中推广应用。

Objective:To evaluate the application of central venous catheter in kidney and perirenal minimally invasive drainage.Methods:The patients of hydronephrosis(53)and renal abscess(32)caused by various reasons in recent 3 years,were collected respectively and randomly divided into two groups.One group was treated by central venous catheter(28;15)and the other were treated by traditional methods(25;17)as control group.Simultaneously,a few of other types of patients(perirenal abscess;hemorrhage of polycystic kidney concurrent infection;malabsorption of perirenal hematocele;renal tuberculous abscess)who treated by central venous catheter were retrospected.Results:Compared with the control group,the patients underwent central venous catheter treatment shows short of operation time,less bleeding,short of postoperation pain time(P﹤0.05).The method of central venous catheter has significant advantage in treatment with hydronephrosis and renal abscess contrast to the traditional method.The treatment effectiveness for other types of patients who treated by central venous catheter were satisfactory.Conclusion:Central venous catheter is deserved to be used in some urology surgery.
