心电图QRS时限与老年急性心肌梗死患者预后的关系 被引量:2

摘要 目的研究心电图QRS时限与老年急性心肌梗死患者预后的关系。方法 160例老年急性心肌梗死患者,根据心电图QRS时限不同分为对照组(QRS时限≤110 ms)和观察组(QRS时限>110 ms),各80例。所有患者均给予常规治疗,并取静脉血送检。比较两组患者N末端脑钠肽前体水平、左心室射血分数(LVEF)、Killip分级及不良心血管事件发生率、心源性死亡发生率。结果两组患者N末端脑钠肽前体、LVEF水平比较,差异有统计学意义(t=5.749、6.166, P=0.000、0.000<0.05)。对照组患者Killip分级Ⅰ级52例,Ⅱ级15例,Ⅲ级13例,Ⅳ级0例,观察组患者Killip分级Ⅰ级25例,Ⅱ级29例,Ⅲ级20,Ⅳ级6例,比较差异有统计学意义(U=3.853, P<0.05)。观察组患者发生恶性心律失常26例,急性肺水肿25例,休克2例,不良心血管发生率为66.25%(53/80),对照组患者发生恶性心律失常8例,急性肺水肿10例,休克8例,不良心血管发生率为32.50%(26/80),比较差异有统计学意义(χ2=18.228,P<0.05)。观察组心源性死亡发生率为23.75%(19/80),明显高于对照组的3.75%(3/80),差异有统计学意义(χ2=13.491, P<0.05)。结论心电图QRS时限与老年急性心肌梗死患者预后有重要关系,老年急性心肌梗死患者心电图QRS时限延长的预后较差,且不良心血管事件及心源性死亡发生率较高,可为临床提供重要参考。
