TNF-α拮抗剂联合营养干预治疗痛风急性发作的疗效观察

Therapeutic Effect of TNF-α Antagonist Combined with Nutritional Therapy on Acute Episode of Gout

目的:探讨TNF-α拮抗剂联合营养干预治疗痛风急性发作的临床疗效。方法:选取2015年2月-2018年6月本院收治的124例痛风急性发作患者。按照随机数字表法将其分为观察组和对照组,各62例。对照组口服秋水仙碱治疗,观察组给予TNF-α拮抗剂联合营养治疗。观察两组治疗前后的炎性反应指标(CRP、IL-1β、ESR、TNF-α)、疼痛情况及缓解时间、复发率和临床疗效。结果:治疗后,两组CRP、IL-1β、ESR、TNF-α水平均低于治疗前,且观察组均低于对照组,比较差异均有统计学意义(P<0.05);治疗后,两组VAS评分均显著低于治疗前,且观察组低于对照组,比较差异均有统计学意义(P<0.05);观察组疼痛缓解率高于对照组,缓解时间短于对照组,复发率低于对照组,比较差异均有统计学意义(P<0.05);观察组总有效率为91.94%,高于对照组的75.81%(P<0.05)。结论:TNF-α拮抗剂联合营养治疗能显著改善痛风急性发作时的炎性反应,加快对疼痛的缓解,疗效显著,是痛风急性发作安全有效的治疗手段。

Objective:To analyze the clinical efficacy of TNF-α Antagonist combined with nutritional therapy for acute episode of gout.Method:A total of 124 patients with acute episode of gout in our hospital from February 2015 to June 2018 were selected.According to the random number table method,they were divided into observation group and control group,62 cases in each group.The control group was treated with Colchicine orally,while observation group was treated with TNF-α Antagonist combined with nutritional therapy.The inflammatoryresponse index(CRP,IL-1β,ESR,TNF-α) before and after treatment,pain and remission time,recurrence rate,and clinical efficacy of two groups were compared.Result:After treatment,the levels of CRP,IL-1β,ESR,TNF-α in two groups were lower than those before treatment,and the observation group were lower than those of control group(P<0.05).After treatment,the VAS scores of two groups were significantly lower than those before treatment,and the observation group were lower than those of control group(P<0.05).The pain relief rate of observation group was higher than that of control group,the remission time was shorter than that of control group,and the recurrence rate was lower than that of control group(P<0.05).The total effective rate was 91.94% in observation group,which was higher than 75.81% in control group(P<0.05).Conclusion:TNF-α antagonist combined with nutritional therapy can significantly improve the inflammatory reaction in acute episodes of gout,accelerate the relief of pain,and has significant curative effect,it is a safe and effective treatment for acute episodes of gout.
