角膜缘干细胞移植用于翼状胬肉治疗的疗效研究

Limbal Stem Cell Transplantation for Pterygium Treatment

目的:研究角膜缘干细胞移植用于翼状胬肉治疗的疗效。方法:选取本院240例(259眼)翼状胬肉患者,按照随机数字表法分为观察组120例(131眼)、对照组120例(128眼)。对照组采取单纯手术切除治疗,观察组在对照组基础上采取角膜缘干细胞方移植治疗。比较两组治愈率、角膜上皮愈合时间、泪膜破裂时间、治疗前后疼痛(VAS)评分及复发情况。结果:观察组治愈率96.95%较对照组88.28%高,角膜上皮愈合时间较对照组短(P<0.05)。观察组术后1、2周泪膜破裂时间较对照组长(P<0.05)。两组术后1周VAS评分比较差异无统计学意义(P>0.05),观察组术后2周VAS评分较对照组低(P<0.05)。随访2年后,观察组复发率3.05%低于对照组19.53%(P<0.05)。结论:翼状胬肉患者采取角膜缘干细胞移植治疗,可显著提高治愈率,缩短角膜上皮愈合时间,强化泪膜稳定性,且疼痛程度较轻,还可明显降低复发率。

Objective:To study the method and clinical superiority of limbal stem cell transplantation for pterygium.Method:240 cases(259 eyes) of pterygium patients in our hospital were randomly divided into observation group 120 cases(131 eyes) and control group 120 cases(128 eyes).The control group was treated with simple surgical excision,while the observation group was treated with limbal stem cell transplantation on the basis of the control group.The cure rate,corneal epithelial healing time,tear film rupture time,VAS score before and after treatment and recurrence were compared between the two groups.Result:The cure rate of observation group was 96.95% higher than that of control group 88.28%, and the healing time of corneal epithelium was shorter than that of control group(P<0.05).After 2 years of follow-up,the recurrence rate of the observation group was 3.05% lower than that of the control group 19.53%(P<0.05).Conclusion:Limbal stem cell transplantation for pterygium can significantly improve the cure rate,shorten the healing time of corneal epithelium, strengthen tear film stability,and reduce the recurrence rate.
