脊柱创伤患者采用传统开放手术和微创手术治疗的对比研究

Effect of Minimally Invasive Surgery and Traditional Open Surgery on Spinal Trauma

目的:探讨脊柱创伤采用传统开放手术和微创手术治疗的临床效果。方法:选取本院2015年12月-2017年12月收治的脊柱创伤患者92例,按照数字随机法将其分为对照组和观察组,各46例。对照组采用传统开放性手术,观察组采用微创手术。比较两组术后疼痛情况(VAS评分)、术中出血量、手术时间、住院时间、下床活动时间,手术前后的Cobb’s角和椎体压缩百分比及并发症发生情况。结果:观察组并发症发生率为6.52%,低于对照组的21.74%(P<0.05);术后,观察组Cobb’s角和椎体压缩百分比均小于对照组,比较差异均有统计学意义(P<0.05);观察组术后VAS评分、术中出血量及住院、下床活动、手术时间均少于对照组,比较差异均有统计学意义(P<0.05)。结论:在脊柱创伤的临床治疗中,运用微创手术,不仅具有操作简单、创伤小、出血量少等优点,还能减少术后并发症,恢复椎体功能,改善患者预后,值得推广。

Objective:To explore the effect of minimally invasive surgery and traditional open surgery on spinal trauma.Method:A total of 92 patients with spinal trauma admitted to our hospital from December 2015 to December 2017 were selected.According to the random number method,they were divided into control group and observation group,46 cases in each group.The control group was treated with traditional open surgery,while the observation group received minimally invasive surgery.The postoperative pain(VAS score),intraoperative bleeding,operation time,hospitalization time,out of bed activity time,Cobb’s angle and vertebral compression percentage before and after operation,and complications between two groups were compared.Result:The complication rate in observation group was 6.52%lower than 21.74%of control group(P<0.05).After operation,the Cobb’s angle and vertebral compression percentage in observation group were smaller than those of control group,the differences were statistically significant(P<0.05).The VAS score,intraoperative bleeding,operation time,hospitalization time,out of bed activity time in observation group were less than those of control group,the differences were statistically significant(P<0.05).Conclusion:In the clinical treatment of spinal trauma,the use of minimally invasive surgery,not only has the advantages of simple operation,less trauma,less bleeding,but also can reduce postoperative complications,restore vertebral function,improve the prognosis of patients,worthy of promotion.
