中药塌渍结合灸法治疗风寒湿痹型膝关节骨性关节炎疗效观察

Effective observation on treating knee osteoarthritis of the Fenghanshibi type by TCM Tazi plus moxibustion

目的:观察中药塌渍结合灸法、温针灸以及常规药物治疗风寒湿痹型膝关节骨性关节炎(KOA)患者的临床疗效差异,探索中药塌渍结合灸法自行居家治疗的可行性。方法:将120例患者随机分为中药塌渍结合灸法组、温针灸组、常规药物组,每组均40例。中药塌渍结合灸法组采用中药塌渍结合特制温灸箱灸治疗,前7次在医院完成,后续则在主管医师的指导下居家治疗。温针灸组采用针刺配合艾灸治疗;常规药物组口服塞来昔布胶囊。各组均每天治疗1次,14次为一个疗程,1个疗程结束后观察疗效。并于治疗前后及随访3个月时对各组进行统计分析。结果:各组VAS评分、关节疼痛积分、膝骨关节病情严重指数评分均低于治疗前(P <0.05),且中药塌渍结合灸法组优于其他两组(均P <0.05)。随访时,中药塌渍结合灸法组的有效率为94.6%,温针灸组为91.7%,常规药物组为85.7%,中药塌渍结合灸法组有效率均优于其他两组(均P <0.05)。结论:中药塌渍结合灸法治疗膝关节骨性关节炎疗效明显,且有操作简单方便、价格低廉等优点,可考虑居家治疗。

Objective:To observe efficacy of TCM Tazi(中药塌渍)plus moxibustion,warm acupuncture and moxibustion,conventional medicine therapy on knee osteoarthritis of the Fenghanshibi type(风寒湿痹型),and to explore the feasibility of TCM Tazi plus moxibustion as a home remedy solution.Methods:120 patients were randomly divided into the TCM Tazi plus moxibustion group,the warm acupuncture and moxibustion group and the conventional medicine group,40 cases in each group.The TCM Tazi plus moxibustion group were treated with TCM Tazi plus moxibustion special stains moxibustion treatment box,the first 7 times were completed in the hospital,followed by home treatment under the guidance of the chief physician.The warm acupuncture and moxibustion group was given acupuncture and moxibustion.The conventional medicine group was given celecoxib capsules.Each group was treated 1 time a day,14 times as a course,and efficiency was observed after 1 course of treatment.Before and after treatment and follow-up 3 months,the related scores and statistical analysis were carried out.Results:In each group,the VAS score,joint pain score,knee osteoarthritis severity index scores were lower than before treatment(P<0.05),and the collapse in the TCM Tazi plus moxibustion group was better than that in other two groups(P<0.05).At the time of follow-up,the efficiency was 94.6%in the TCM Tazi plus moxibustion group,91.7%in the warm acupuncture and moxibustion group,and 85.7%in the conventional medicine group.The efficaty in the TCM Tazi plus moxibustion group was better than that in other groups(P<0.05).Conclusion:TCM Tazi plus moxibustion on knee osteoarthritis shows obvious effects,and has the advantages of simple and convenient operation,low price,can be considered the treatment at home.
