中药熏洗联合Q-1064激光治疗难治性局限性神经性皮炎疗效探讨

Efficacy of TCM fumigation plus Q-1064 laser on refractory localized neurodermatitis

目的:探讨中药熏洗联合Q-1064激光治疗难治性局限性神经性皮炎疗效。方法:我院门诊确诊为难治性局限性神经性皮炎患者80例,随机分为联合治疗组(中药熏洗+Q-1064激光)和激光治疗组(Q-1064激光)各40例。激光治疗组采用Q-1064激光治疗,2周治疗1次;联合治疗组在激光治疗基础上加用中药熏洗治疗,2次/d,20min/次,1剂/2d。两组均以10d为一个疗程,治疗4～8个疗程。分别观察4个疗程和8个疗程结束后的疗效情况,治疗前后皮损面积和瘙痒程度评分,及停药6个月后复发率,记录不良反应。结果:治疗4个疗程和8个疗程后,联合治疗组总有效率均明显高于激光治疗组,差异均有统计学意义(P <0.05)。治疗4个疗程后,联合治疗组皮损面积和瘙痒程度评分比激光治疗组降低,治疗8个疗程后皮损面积和瘙痒程度评分比激光治疗组降低,差异均具有统计学意义(P <0.05)。停药6个月后根据随访调查情况,联合治疗组复发率明显低于激光治疗组,差异具有统计学意义(P <0.05)。结论:治疗难治性局限性神经性皮炎采用中药熏洗和Q-1064激光治疗联合疗法疗效较好且复发率低。

Objective:To explore the efficacy of TCM fumigation plus Q-1064 laser on refractory localized neurodermatitis.Methods:80 cases in outpatient department of our hospital were randomly divided into the combined treatment group(TCM fumigation plus Q-1064 laser,n=40)and the laser treatment group(Q-1064 laser,n=40).The laser treatment group was given Q-1064 laser and treated twice a week,and the combined treatment group was additionally given TCM fumigation(2 times/d,20min/time,1 dose/2d)more.Two groups were treated for 4 to 8 courses as 10d for one course.The efficacy,area of skin lesions and score of pruritus degree before and after treatment,and the recurrence at 6 months after discontinuation were observed after 4 courses and 8 courses of treatment,and the adverse reactions were recorded.Results:After 4 courses and 8 courses of treatment,the total efficiency in the combined treatment group was significantly higher(P<0.05).After 4 courses of treatment,the area of skin lesions and score of pruritus degree in the combined treatment group were lower,and the indexes were significantly lower than those in the laser treatment group after 8 courses of treatment(P<0.05).At 6 months after discontinuation,4 cases were recurred among 17 cured patients in the combined treatment group and 9 cases were recurred among 12 cured patients in the laser treatment group according to the follow-up survey,and the recurrence rate in the combined treatment group was significantly lower than that in the laser treatment group(P<0.05).Conclusions:TCM fumigation plus Q-1064 laser for refractory localized neurodermatitis show a good efficacy and low recurrence rate.
