纤维乳管镜及癌胚抗原在乳头溢液中的临床应用研究

Clinical Application of Fiberoptic Ductoscopy and CEA in Nipple Discharge

目的临床研究纤维乳管镜及癌胚抗原在乳头溢液中的的应用。方法选取53例有单侧乳头溢液的女性患者,均未接受术前治疗。所有的患者行乳管镜检查,若未探查见明显肿物,行保守治疗,若探查见肿物行手术治疗,根据术后病理诊断分为乳腺癌组和导管内乳头状瘤组。比较单纯的乳头溢液CEA的测定、乳管镜、乳头溢液CEA测定与乳管镜联合应用的准确率。结果单纯的乳头溢液CEA检测、乳管镜检查都具有较高的诊断率,但两者综合使用诊断率可达100%。乳头溢液CEA的测定在导管内无肿物、良性肿物、恶性肿物的数值不同。结论乳头溢液CEA检测联合乳管镜检查可使诊断率大大提高。

Objective To study the application of fiberoptic ductoscopy and carcinoembryonic antigen in nipple discharge.Methods 53 cases of female patients with unilateral nipple discharge were not treated before operation.All the patients were treated with endoscopy.If the tumor was not detected,the patients were treated with conservative treatment.If the tumor was found to be operated on,the patients were divided into breast cancer group and intraductal papilloma group according to the postoperative pathological diagnosis.The accuracy of CEA in the diagnosis of nipple discharge,fiberoptic ductoscopy,and the accuracy of combined fiberoptic ductoscopy and nipple discharge CEA measurement were compared.Results Simple CEA detection of nipple discharge and breast ductal endoscopy had higher diagnostic rate,but the combined diagnosis rate was 100%.The measurement of CEA in nipple discharge was different from that in the catheter without tumor,benign tumor and malignant tumor.Conclusion CEA detection of nipple discharge and combined with ductal endoscopy can greatly improve the diagnostic rate.
