彩色多普勒超声与CTA在评价糖尿病下肢动脉病变中的对比研究

A comparative study of Color Doppler Ultrasonography and CTA in the evaluation of diabetic lower extremity artery disease

目的对比彩色多普勒超声与CT血管成像(Computed Tomography Angiography,CTA)对糖尿病下肢动脉病变的诊断价值。方法回顾性分析2009年1月至2017年12月本院收治的糖尿病并发下肢动脉狭窄或闭塞的90例患者的下肢动脉彩色多普勒超声检查及CTA检查影像资料,与下肢动脉血管造影(DSA)结果对比,分析两种检查方法的准确度、灵敏度、特异度。结果 DSA显示90例患者下肢动脉不同程度病变1级352段,2级313段,3级67段,4级49段,5级17段。彩色多普勒超声检查和CTA检查对下肢动脉不同程度狭窄及闭塞诊断的准确度比较,差异均无统计学意义;彩色多普勒超声检查和CTA检查对下肢动脉不同程度狭窄诊断的灵敏度比较,两者差异无统计学意义,两者对下肢动脉闭塞诊断的灵敏度差异有统计学意义(P<0.05);彩色多普勒超声检查和CTA检查对下肢动脉不同程度狭窄及闭塞诊断的特异度比较,差异均无统计学意义。结论彩色多普勒超声检查和CTA检查都能够准确直观地显示糖尿病患者下肢动脉狭窄、闭塞,为临床医师对疾病的诊断和治疗提供更多依据,对糖尿病下肢动脉病变具有较高诊断价值。

Objective To compare the diagnostic value of Color Doppler Ultrasound and CT angiography(CTA)CTA in diabetic lower extremity artery disease.Methods A retrospective analysis of 90 patients with diabetes mellitus complicated with lower extremity artery stenosis or occlusion from January 2009 to December 2017 was made.The imaging data of lower extremity artery Color Doppler Ultrasonography and CTA were compared with the results of lower extremity artery angiography(DSA).The accuracy,sensitivity and specificity of the two methods were analyzed.Results DSA showed that there were 352 segments in grade 1,313 segments in grade 2,67 segments in grade 3,49 segments in grade 4 and 17 segments in Grade 5 in 90 patients.There was no significant difference between Color Doppler Ultrasonography and CTA in the diagnosis of lower extremity artery stenosis and occlusion.There was no significant difference between Color Doppler Ultrasonography and CTA in the diagnosis of lower extremity artery stenosis and occlusion.The sensitivity of occlusion diagnosis was significantly different(P<0.05);there was no significant difference between Color Doppler Ultrasonography and CTA in the diagnosis of lower extremity artery stenosis and occlusion.Conclusion Both Color Doppler Ultrasonography and CTA can accurately and intuitively show the stenosis and occlusion of lower extremity arteries in diabetic patients,which can provide more evidence for the diagnosis and treatment of diseases by clinicians,and has high diagnostic value for lower extremity arterial diseases in diabetes.
