本院皮肤科门诊处方及合理用药研究

Study on out-patient prescriptions and rational drug use in department of dermatology in our hospital

目的对2017年至2018年本院皮肤科门诊处方是否合理进行调查,为合理用药提供借鉴经验。方法选取本院皮肤科门诊2017年至2018年开出的6 200张处方进行检查,对药物名称、用药剂量、用药方式、药物配伍等进行考察,根据世界卫生组织(World Health Organization简称WHO)推荐的药物使用剂量、药品说明书对处方合理性进行判定,记录调查结果。结果 6 200张处方中有72张处方存在用药不合理现象(1.16%)。皮肤科处方中,抗组胺类药物使用频率最高,如西替利嗪片、氯雷他定片等,其次激素类、中成药、免疫调节剂使用频率也较高。处方不合理原因主要为药物配伍不合理、用药时间过长、用量不准确等。结论本院皮肤科门诊处方存在不合理现象,门诊医师应对皮肤科药物加强管理,明确药物使用适应症,熟知药物配伍知识、不良反应,对患者进行药物使用知识宣传教育,减少不合理处方,保证药物的使用安全及使用效果。

Objective To investigate the rationality of outpatient prescriptions in department of dermatology in our hospital in the past 2017-2018 years,so as to provide reference for rational drug use.Methods 6 200 prescriptions in the outpatient department of the Department of Dermatology of our hospital were selected from 2017 to 2018.The drug name,dosage,the way of drug use,and the compatibility of the drugs were investigated.The rationality of the prescription was determined according to the recommended dosage of WHO and the drug instruction,and the results of the investigation were recorded.Results Among the 6 200 prescriptions,72 prescriptions were unreasonable in drug use(1.16%).In Department of Dermatology prescriptions,antihistamines have the highest frequency of use,such as cetirizine,Loratadine Tablets and so on,followed by high frequency of hormone,Chinese patent medicine and immunomodulator.Unreasonable prescriptions are mainly due to unreasonable drug compatibility,too long medication time and inaccurate dosage.Conclusion The outpatient prescriptions in Department of dermatology in our hospital are unreasonable.The out-patient doctors should strengthen the administration of drugs in the Department of Dermatology,make clear the indications of drug use,know the knowledge of drug compatibility and adverse reactions,carry out the knowledge and education of drug use to the patients,reduce the irrational prescriptions,and ensure the safety and effect of the use of the drugs.
