彩色多普勒超声与多层螺旋CT扫描在颈部淋巴结结核诊断中的应用对比分析

The comparative analysis of Color Doppler Ultrasonography and Multi-slice CT scan in the diagnosis of cervical lymph node tuberculosis

目的对比分析彩色多普勒超声检查与多层螺旋CT扫描对颈部淋巴结结核的诊断差异。方法选取2014年1月至2016年12月深圳市慢性病防治中心及深圳市第三人民医院收治的40例病理确诊为颈部淋巴结结核的患者,同期均予以双侧颈部彩色多普勒超声检查及多层螺旋CT扫描,回顾性分析其临床影像资料,观察其影像表现,比较两种检查方法对颈部淋巴结结核检出率,计算两种方法的灵敏度、一致率及Kappa值。结果 40例患者中,彩色多普勒超声检查诊断颈部淋巴结结核的患者27例,准确率67.5%;CT诊断颈部淋巴结结核的患者28例,准确率70.0%;超声检查与CT扫描的诊断准确率差异无统计学意义(P>0.05)。两种方法联合诊断颈部淋巴结结核患者36例,准确率90.0%。结论颈部淋巴结结核的彩色多普勒超声声像图和多层螺旋CT影像表现均具有一定的特征性。CT扫描对颈部淋巴结结核诊断准确率略高于超声检查,超声联合CT增强扫描可明显提高颈部淋巴结结核诊断准确率。

Objective To compare the two methods of Color Doppler Ultrasonography and Multislice Computed Tomography scans in the diagnosis of cervical lymph node tuberculosis.Methods Forty patients diagnosed with cervical lymph node tuberculosis which confirmed by pathology from January 2014 to December 2016 in Shenzhen Center for Chronic Disease Control and Shenzhen third people's hospital were selected as the research objects.And the patients were both diagnosed with Color Doppler Ultrasonography and Multi-slice Computed Tomography scans at the same time.All patients'imaging features were retrospectively analyzed.Accuracy rate of cervical lymph node tuberculosis and imaging findings of the two methods were compared.The sensitivity,concordance rate and kappa value of the two methods were calculated.Results In 40 patients,27 cases with cervical lymph node tuberculosis were diagnosed by the Color Doppler Ultrasonography,which accounted for 67.5%of all patient;and 28 cases were confirmed by Multi-slice Computed Tomography scans,which accounted for 70.0%.The difference of two methods showed no statistical significance(P﹥0.05).The accuracy rate of the two approaches combined method was 90.0%.Conclusion Color Doppler Ultrasonography and Multi-slice Computed Tomography scans show distinguishable characteristics in the detection of cervical lymph node tuberculosis.Contrast-enhanced CT scanning has higher accuracy than Color Doppler Ultrasonography.Ultrasounography combined with enhanced CT scan can improve the accuracy of diagnosis of cervical lymph node tuberculosis obviously.
