摘要
目的探讨父母小组培训对注意缺陷多动障碍(ADHD)患儿药物治疗依从性的影响。方法将首次诊断为ADHD且就诊前未接受过治疗的350例患儿随机分为两组,干预组(父母小组培训)178例和对照组(父母门诊简单宣教)172例。对父母干预后,两组患儿均予盐酸哌甲酯缓释片口服治疗6个月。比较干预前后两组患儿父母对ADHD、药物治疗的认识及亲子互动状况,并比较两组患儿接受药物治疗比例及依从性。结果干预前,两组患儿父母对ADHD、药物治疗的认识及亲子互动状况比较,差异均无统计学意义(均P>0.05);干预后,干预组患儿父母对ADHD、药物治疗的认识及亲子互动状况明显改善(P<0.05),且优于对照组(P<0.05);对照组干预前后比较,差异均无统计学意义(均P>0.05)。干预组患儿接受药物治疗的比例为56.7%,明显高于对照组的29.1%(P<0.05);药物治疗依从率为94.1%,亦明显高于对照组的60.0%(P<0.05)。结论父母小组培训有助于提高父母对ADHD及药物治疗的认识,增进亲子互动,提高药物治疗的依从性。
Objective To evaluate the effect of parental group training on the drug therapy compliance in children with attention deficit hyperactivity disorder(ADHD). Methods Three hundred and fifty children with initially diagnosed ADHD without prior treatment were randomly divided into two groups, the intervention group(n=178), their parents received group training and the control group(n=172), their parents received conventional health education. The children from both groups were treated with methylphenidate hydrochloride extended-release tablets for 6 months. The parents’ knowledge about ADHD and drug therapy, parent-child interaction and therapeutic compliance of children were compared before and after intervention between the two groups. Results Before intervention, there were no significant differences in the parents’ knowledge about ADHD and drug therapy, parent-child interaction between the two groups(P >0.05).After intervention, the parents’ knowledge about ADHD and drug therapy and the parent-child interaction in the intervention group were significantly improved(P<0.05), and better than those in the control group(P<0.05). while there was no significant difference in the control group before and after intervention(P >0.05). The proportion of the children receiving drug therapy in the intervention group was significantly higher than that in the control group(56.7% vs 29.1%, P<0.05). The compliance rate of drug therapy in the intervention group was significantly higher than that in the control group(94.1% vs 60.0%, P <0.05). Conclusion The parent group training can enhance the parents’ knowledge of the disease and its therapy, improve parent-child relationship and increase the compliance of drug therapy for children with ADHD.
