钼靶联合超声BI-RADS对乳腺原位癌的诊断价值分析 被引量:9

摘要 目的探讨钼靶联合超声BI-RADS诊断原位乳腺癌的临床价值。方法将手术治疗且经病理诊断证实为乳腺原位癌的79例患者作为研究组,随机选取同期接受诊治的乳腺良性疾病患者53例作为对照组,观察比较2组患者肿块的影像学特征、钼靶分级诊断和BI-RADS超声诊断结果。结果 2组患者的乳腺肿块边界、形态、生长方式和内部回声等影像特征比较,差异有统计学意义(P <0.05)。单纯使用BI-RADS超声诊断的阳性率为82.28%,使用钼靶分级诊断阳性率为70.89%,联合诊断阳性率为94.94%,诊断准确率三者间差异有统计学意义(P <0.05)。超声分级诊断中4级敏感度为43.04%,明显高于钼靶分级;而钼靶分级2级和3级的敏感度分别为9.43%和15.09%,明显高于超声分级,差异具有统计学意义(P <0.05)。结论 BI-RADS超声与钼靶分级联合应用可有效地提高乳腺原位癌诊断的准确率和敏感度,检查结果更为客观准确,在早期乳腺原位癌的诊断中具有重要的意义。 Objective To investigate the clinical value of molybdenum target combined with ultrasound BI-RADS in the diagnosis of breast cancer in situ. Methods 79 patients who had undergone surgical treatment and pathological diagnosis confirmed breast cancer in situ were used as study group.53 patients with benign breast disease who were diagnosed and treated at the same time were randomly selected as the control group.The imaging characteristics of tumor,grading of molybdenum target and the results of BI-RADS ultrasonography were observed and compared between the two groups. Results There were statistically significant differences in the imaging characteristics of the breast mass,such as borders,morphology,growth patterns,and internal echo in the two groups of patients( P <0.05).The positive rate of BI-RADS ultrasonography was 82.28%,the positive rate of using molybdenum target grading was 70.89%,and the positive rate of combined diagnosis was 94.94%,the difference in diagnostic accuracy was statistically significant ( P <0.05).The grade 4 sensitivity in ultrasonic grading diagnosis was 43.04%,Which was significantly higher than that of molybdenum target.The sensitivity of grade 2 and 3 of molybdenum target was 9.43% and 15.09% respectively,which was significantly higher than that of ultrasonic classification( P <0.05). Conclusion The combined application of BI-RADS and mammography can effectively improve the accuracy and sensitivity of the diagnosis of orthotopic breast cancer.The results are more objective and accurate,which is of great significance in the diagnosis of early-stage orthotopic breast cancer.
