外周血循环肿瘤细胞和肿瘤标志物在监测转移性乳腺癌治疗及预后中的价值

The Value of Detection of Peripheral Blood Circulating Tumor Cells and Tumor Markers in the Treatment and Prognosis of Metastatic Breast Cancer

目的探讨外周血循环肿瘤细胞和肿瘤标志物在监测转移性乳腺癌疗效及预后中的价值。方法选取50例患有转移性乳腺癌疾病的患者,均接受化疗、内分泌治疗以及靶向治疗,检测50例患者在转移性乳腺癌治疗时的外周血循环肿瘤细胞(CTCs)值以及CTC阳性率变化情况;同时再选取50例患有转移性乳腺癌患者的血清,采用电化学发光法检测血清肿瘤标志物在治疗前后的表达水平。结果各检测方法均有差异,医院应结合自身水平和患者具体情况采取恰当的方式进行外周血循环肿瘤细胞的检验;治疗前的CTC数值和阳性率明显高于治疗后的数值;治疗前CA125和CA153水平均明显高于治疗后,其他四项(CEA、AFP、PSA、CA199)则无明显差异。结论外周血循环肿瘤细胞检测和肿瘤标志物的检测既能评估患者的疗效及患者的预后,又可作为分析患者肿瘤生物学特征的实时样本,及时检测能够有效地帮助转移性乳腺癌的治疗过程,提高治疗效果。

Objective To investigate the value of peripheral blood circulating tumor cells and tumor markers in monitoring the efficacy and prognosis of metastatic breast cancer. Methods 50 patients with metastatic breast cancer were treated with chemotherapy,endocrine therapy and targeted therapy.The changes of peripheral blood circulating tumor cells (CTCs) and CTC positive rate in 50 patients with metastatic breast cancer were detected.At the same time,50 patients with metastatic breast cancer were selected,and the expression levels of serum tumor markers before and after treatment were detected by electrochemiluminescence. Results The detection methods were different,the hospital should be tested according to their own level and the actual situation of patients to take appropriate way to test peripheral blood circulating tumor cells;tThe CTC value and positive rate before treatment were significantly higher than the values after treatment;the levels of CA125 and CA153 before treatment were significantly higher than those after treatment,and the other four items (CEA,AFP,PSA,CA199) had no significant difference. Conclusion The detection of circulating tumor cells and tumor markers in peripheral blood can evaluate curative effect and prognosis of patients,and can be used as real time samples of patients with tumor biological characteristics analysis,timely detection can effectively help the treatment of metastatic breast cancer,improve treatment effect.
